Assists in the updating of the fall prevention intervention log (which includes visual interventions. K. Meets with new admissions and reviews primary safety measures. L. Reports any changes in resident/patient condition to the nurse on duty. M. Secures incident/accident reporting from the nurses upon arrival for duty. N. Checks to ensure that new interventions are promptly implemented post occurrence. O. Communicated trends/patterns, and individual resident/patient safety concerns to the IDT through Stand-Up Meeting (day and evening shifts), and during early mornings for night shift. P. Communicates with the CNAs to assists with determining the root cause or new behaviors associated with incidents/accidents, and potential new interventions based on the root cause. Communicates this information to the restorative nurse. Q. Meets with DON and corporate to review observations and recommendations for the incident/accident prevention program. R.
