Ensures all pre-visit work required for exam is completed and that lab and other test results are available in the electronic medical record (EMR). You will also provide direct clinical support to one or more assigned clinicians. Primary Responsibilities: Provides efficient flow of patients to optimize patient appointment availability Greets and escorts patients to exam rooms according to policy and procedures Prepare patient for examination; Measures vital signs, such as blood pressure, pulse rate, weight, and height Assist clinicians with routine clinical procedures and/or additional tests specific to the department. Advises patients of preparation or pre-visit testing required for exams specific to the department Ensures patients' electronic medical record (EMR) is updated with medical information, including recent testing and consultations, including confirming demographic information, medications, allergies and chief medical complaints, and ensuring the EMR patient data is accurate and up to date Keeps patients informed of visit status, unforeseen delays and other relevant information Reconcile medications and sets up pending refills. Schedules and Completes Minimum Data Set (MDS) Assessments and care plans for all residents assigned . 2. Monitors completion of MDS Assessments by other disciplines and ensures timely completion per CMS and RAI guidelines and Signs off the RN completion. 3. Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing. 4. Ensures accurate, timely completion of the MDS/CAAs/Care plans sheet for assigned residents. 5. Initiates care plans and supporting activities that will result in best possible outcome for assigned residents. 6. Generates and distributes monthly care plan calendar for the following month. 7. Conducts care plan conferences for assigned residents. 8. Maintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues, as necessary. 9. Reviews all resident incidents and accidents and the 24-hour report daily to ensure that care plans for assigned residents reflect current and changing needs. 10. At patient care conferences, facilitates, and coordinates the activities of the disciplines of nursing, rehabilitation, dietary, activities, social services, and restorative nursing. 11. Monitors and ensures completion of 7-day look-back documentation by nursing staff within prescribed MDS timeframe. 12. Participates in facility management meetings as requested. 13. Monitors and audits clinical record documentation on assigned residents to ensure accuracy and timeliness. 14. Advises Director of Nursing Care of persistent issues related to non-compliant documentation on assigned residents. 15. Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current / new regulations. 16. Maintains current knowledge of RAI Manual and MDS PDPM assessment process. 17. Attends in-service training and other educational programs as directed or authorized. 18. Performs all job duties in a manner that ensures that confidential information and resident rights are protected at all times. 19. Supports quality of care and operational efficiency by maintaining acceptable attendance record. 20. Performs all job duties in a safe manner in accordance with established safety guidelines, protocols and expectations. 21. Reports unsafe conditions when noted and immediately reports accidents to immediate supervisor or another designated on-duty manager. 22. Supports and promotes facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers. 23. Complies with established standards described in facility policies and procedures, code of conduct, corporate compliance plan, employee handbook and other company documents and publications. Job Summary We are seeking a part-time, 25-hour Medical Assistant to join our Express Care team! Express Care, formally known as Urgent Care, offers timely treatment for non-emergency conditions. We care for adults and children ages 3 and up, addressing a wide range of issues such as respiratory illnesses, infections, minor injuries, rashes, and stomach problems. Because patients often visit when they’re feeling unwell, our team prioritizes compassion and emotional support alongside quality medical care. The schedule consists of working 2, 12.5-hour shifts per week (8:30am to 9:00pm), including every other weekend (both Saturday and Sunday). Since Express Care is a 7-day-per-week operation that delivers care during the day, evening, weekends, and holidays, the schedule for this role will require 2 on-call shifts per 4-week scheduling period, weekend, holiday coverage and the flexibility to work overtime as needed. Job Summary The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department's Clinical Practice Manual. The registered nurse demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation and evaluation of patient care and is directly responsible to a designated nurse manager, or supervisor. Yes Essential Functions: -Organization of Patient Care • Accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment. • Sets priorities when organizing care for patients with varying acuity. • Maintain continuity through clear and concise (verbal and written) communication. • Demonstrates appropriate knowledge of growth and development of the adult and geriatric patient. In areas, such as the NICU and Obstetrics the principals of growth and development are applicable to the neonate. -Quality of Care • Documents patient care in a manner that is clear, complete, concise and in compliance with nursing documentation standards. • Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records. • Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department’s documentation standards. • Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources. • Evaluates the effectiveness of the plan of care, and documents progress in meeting stated goals. Revises plan of care as needed to achieve desired outcomes. • Participates actively in the unit-based and organizational quality management and/or quality improvement programs. • Provides a safe environment for patients, staff, family and visitors. • Administers medication safely according to established policies and procedures. • Performs nursing procedures safely and efficiently. • Uses equipment safely and efficiently. • Demonstrates awareness of potential/actual risks of infection and modes of transmission. • Utilizes universal precautions in nursing practice. -Coordination/Collaboration • Interacts with patients, families, and colleagues in a professional manner. • Collaborates with other discipline(s). • Develops, utilizes and evaluates unit-specific standards of care. • Upholds the A.N.A. code of ethics and acts as a role model to other staff members. • Participates in determining and implementing goals and objectives for the unit. • Participates in determining goals and objectives in the periodic review and evaluation. • Serves on unit-based and organizational committees and disseminates information to peers. -Education • Assumes responsibility for personal and professional growth through identification of own learning needs. • Shares knowledge and experience with colleagues. • Participates in unit-based and organizational educational programs. • Seeks and accepts guidance for additional learning needs. -Research • Utilizes nursing research findings in clinical practice. • Contributes to nursing and/or medical research endeavors by supporting investigators. -Budget • Utilizes time and materials in an effective and economical manner. • Provides suggestions which support the delivery of cost-effective health care. • Assesses patient's acuity accurately when making decisions regarding staffing, transfers and assignments. • Demonstrates awareness of the need to manage within established budgetary boundaries. -Personnel • Participates in the orientation of new staff members, students and others as appropriate. • Provides input into the clinical evaluation of other staff, as appropriate. • Assumes responsibility for the unit in the absence of leadership personnel. • Adheres to hospital and nursing policies and aids in their interpretation to others. • Recommends change in policies and procedures through appropriate channels. • Participates in the cooperative effort and peer support required for the smooth running of the unit, e.g., flexibility in relation to patient assignments, shift assignment, or work schedule.
