Overview Manages the development, operations and quality of the Nurse Family Partnership (NFP) team, ensuring fidelity to the NFP model is consistent with NFP national and Department of Health and Mental Hygiene (DOHMH) standards, and VNS Health philosophy, policies, goals and objectives, and Standards of Nursing Practice. Ensures that NFP and VNS Health standards for service delivery and team performance are met or exceeded. Uses principles of supervision, reflective practice, and staff development, to provide primary support and appropriate oversight to the nurse home visitors, and to build and model a strengths-based, client-centered culture. Monitors compliance with NFP, DOH and VNS Health goals and objectives, and develops team strategies for improvement. Collaborates with the DOHMH and agency programs and departments to design and implement marketing and customer relation initiatives. Promotes NFP Program through liaison, lecturing and promotional activities in the community. Manages service delivery of assigned NFP team; ensures achievement of team goals and performance measures within the NFP and Agency’s strategic objectives and assumes accountability for achievement of team’s clinical, process and fiscal performance outcomes. Observes and analyzes performance patterns and uses reflective supervision to develop interventions to improve individual and team performance. Provides guidance and support to assigned team to encourage and support team learning, implementation and evaluation of plans that maintain and increase customer response, cost-efficiency, and quality care compliant with NFP and regulatory standards. Ensures that all staff, including supervisor, attend all required NFP Initial Education sessions as outlined in the orientation materials and contract with the Nurse-Family Partnership National Service Office. Acts as a key resource and/or liaison in providing clinical and operational guidance and support to team. Uses NFP, DOHMH and VNS Health resources to achieve and enhance team outcomes. Supports, collaborates and provides guidance to NFP nurses in working through team development issues. Develops staff through individual and team coaching, use of internal and external resources and participation in NFP education and training sessions to build confidence and skill of nurse home visits. Involves staff in the process of self-development and holds staff accountable for meeting performance expectations and development plans. Uses a variety of methods in obtaining information about team member’s performance (e.g., team meetings, home visits, frequent chart review, and feedback from other professionals and clients). Analyzes and helps to resolve complex clinical case management problems, working in collaboration with other NFP managers, clinical specialists, and other VNS Health professional staff, as appropriate. Provides guidance to individuals and team to identify and resolve internal and external customer issues. In consultation with team members, identifies staffing and service needs of designated geographic areas and recommends changes as needed. Interprets and implements NFP/DOHMH and agency policies, state and federal regulations, and collective bargaining agreements. Collaborates with other NFP team members to develop and maintain relationships with community health and social service providers to ensure reciprocal referral relationships that support NFP enrollment requirements and access to resources that meet family needs. Performs all duties inherent in a managerial role. Ensures effective staffing, evaluates performance, provides input for the development of the NFP budget, and hires, promotes, and terminates staff and recommends salary actions, as appropriate. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Overview Provides licensed practical nursing support to the Medical Care at Home (MCAH) clinical team during in-house and virtually supported preventive assessments. The LPN, Field Clinical Support is physically present with the patient and works under the direction of the Nurse Practitioner (NP), collecting and communicating objective clinical information, completing structured intake activities, supporting Care GAP closure, performing ordered procedures and testing within LPN scope and demonstrated competency, and reinforcing patient education. The NP maintains responsibility for the clinical assessment, interpretation of findings, diagnoses, Assessment and Plan, and final encounter documentation. • Prepares patients for MCAH preventive and comprehensive assessment visits and completes structured intake activities, including collection of medical history, medication information, vital signs, functional information, and other objective clinical data for NP review. • Works under the direction of the NP during virtual encounters and communicates objective findings, changes in patient condition, and other clinical observations requiring NP review. • Supports medication reconciliation by comparing available medication information with patient-reported use, identifying discrepancies, and communicating findings to the NP for assessment and follow-up. • Performs ordered clinical procedures, screenings, specimen collection, point-of-care testing, and other nursing activities that are within LPN scope, organizational policy, and demonstrated competency. • Performs phlebotomy and ensures specimens are collected, labeled, handled, stored, and transported according to established protocols when applicable. • Supports completion of identified Care GAPs by reviewing assigned preventive needs, facilitating ordered testing, documenting completion, and communicating barriers or outstanding needs to the NP and appropriate team members. • Provides NP-directed examination support and reports objective clinical findings in real time during the virtual encounter. When further examination cannot be appropriately completed during the visit, supports coordination of follow-up as directed by the NP. • Recognizes and promptly reports abnormal findings or changes in patient condition to the NP.
