Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Responsibilities Patient Care Leading nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. Validates staff members round hourly on all patients by direct observation and patient feedback. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. Conducts rounding of all patients on a daily basis (Monday-Friday.) Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. Collaborates with the physician dyad and escalates concerns as appropriate. Follows up on all patient concerns in a timely manner and provides service recovery as needed. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Huddles with staff at the start of each shift, identifying high-risk patients who require special attention. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Associate Director of Nursing/designee or Patient Relations as needed. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. Empowers staff to use evidence-based practices to increase understanding of patient's perceptions of care. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Quality and Safety Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. Leads interdisciplinary quality improvement teams using the principles of high reliability. Reviews unit progress, changes and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and the healthcare team to promote patient care and regulatory standards. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Maintains control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Monitors availability and maintenance of nursing equipment and supplies. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision-making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Interprets and implements collective bargaining agreements affecting staff. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. Professional Development Performs as a clinical leader who actively creates and supports nursing practice and an environment of care that reflects clinical excellence. Organizes the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. Qualifications Bachelor of Science with a major in nursing required, master's in nursing or health related field preferred. 3 years minimum Emergency Department (ER) clinical nursing experience with some Charge Nurse/Supervisory experience preferred. Must have relevant clinical competence in area of nursing practice assigned. Licensed as a registered nurse with current registration in New York State. ACLS, BCLS, PALS from AHA required. GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program. Daily Visits Initiates and review orders, including medications, on a daily basis Reviews labs, radiology reports, and consults on all patients Talks to and examines each assigned skilled-level patient on daily rounds Monday through Friday Writes at least one daily progress note for each skilled patient Assess patient’s medical stability daily. Consults/coordinates with specialists as needed Addresses acute mental status changes via non-pharmacologic or pharmacologic measures, consultation or transfer Coordinates/assess rehab progress on a daily basis Discusses concerns with the patient, family, rehab, and case management. Educates patient and family members regarding acute and chronic illness management Attends family meetings as necessary Assists PCP’s that participate in SNF management Informs attending and/or ACA medical director of significant changes in medical condition Participates in weekly utilization meetings, collaborating with the SNF care team and ACA care managers Coordinates with PCP’s, Hospitalists, ACA Medical Directors and Case Managers Performs home visits on selected patients Addresses /coordinates any legal issues. Discharge Develops a discharge plan utilizing input from case management and rehab. Identify barriers to discharge Creates a detailed discharge summary for each admission on all patients, including medication reconciliation, and sends to the PCP at the time of SNF discharge Ensures that patients have all appropriate drug and DME prescriptions at discharge Coordinates visits with the PCP post-discharge Discharges summary to be sent to the PCP at discharge Updates all patients in Care Screen™ before discharge Coordinates transition from skilled to long term placement. Long-Term Care Assists case management in the evaluation of selected long term patients Follows “new” long term patients every 30 days Assists the attending physician with management for complex long-term patients Qualifications EDUCATION AND EXPERIENCE License to practice as a Registered Nurse and a certificate to practice as a Nurse Practitioner issued by the State Board of Registered Nursing. Geriatrics specialty certification preferred Minimum of three years of clinical nursing experience preferred, including work in a skilled nursing facility. 20 days PTO, Health insurance, 401 k %2, Malpractice insurance.
