Contract Details · Contract Length: 4 weeks · Position Type: Temporary / Per Diem Contract · Minimum Hours: 40 hours per week · Weekly Compensation: $1,600 per week, less applicable taxes and withholdings · Schedule: Flexible scheduling based on facility needs; must be available to work a minimum of 40 hours per week · Weekend Requirement: Every other weekend required · Shift: Day, evening, or overnight shifts based on facility needs and availability · Overtime: Subject to facility policy and applicable federal and state wage laws · Contract Completion: Assignment concludes at the end of the four-week term unless an extension is mutually agreed upon Essential Duties and Responsibilities Resident Care · Provide direct nursing care to residents in accordance with established care plans, physician orders, and facility policies. · Administer medications and treatments accurately and safely. · Monitor residents for changes in condition and promptly report significant findings to the RN, provider, and appropriate members of the interdisciplinary team. · Perform treatments, wound care, assessments, and other nursing interventions within the LPN's scope of practice. · Assist with admissions, transfers, discharges, and other resident-care processes as assigned. · Maintain accurate and timely documentation in the electronic medical record. · Promote resident dignity, privacy, independence, and quality of life. Clinical Responsibilities · Follow all physician orders, nursing standards, facility policies, and applicable regulatory requirements. · Complete required assessments and documentation within established timeframes. · Participate in care planning and communicate relevant resident information to the interdisciplinary team. · Recognize and respond appropriately to changes in resident condition. · Follow infection prevention and control procedures. · Maintain medication and treatment areas in accordance with facility standards. · Participate in emergency response procedures as required. Teamwork and Communication · Work collaboratively with CNAs, RNs, providers, rehabilitation staff, social services, dietary, activities, and other departments. · Provide appropriate direction and support to nursing assistants within the LPN's scope of responsibility. · Communicate professionally with residents and families. · Maintain a respectful and positive working relationship with facility staff. · Participate in shift report and ensure appropriate communication of resident needs. Minimum Qualifications · Current, unrestricted LPN/LVN license in the state of employment. · Graduate of an approved practical nursing program. · Current CPR certification, if required by facility policy. · Previous long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred. · Ability to work a minimum of 40 hours per week throughout the four-week contract. · Ability to work assigned shifts, including weekends as required. · Ability to successfully complete required employment screening and facility onboarding requirements. Performance Expectations Contract LPNs are expected to: · Maintain reliable attendance and punctuality. · Complete all scheduled shifts unless appropriately excused. · Provide safe, competent, and compassionate resident care. · Follow all facility policies and procedures. · Maintain professional conduct at all times. · Complete documentation accurately and timely. · Communicate changes in resident condition promptly. · Maintain confidentiality of resident and facility information. · Meet all applicable state and federal nursing requirements. Contract Agreement The LPN acknowledges that this is a four-week temporary contract assignment requiring a minimum of 40 hours per week . Compensation is $1,600 gross per week, less applicable taxes and withholdings , based on completion of the required minimum weekly hours and satisfactory performance. Continuation beyond the four-week term is not guaranteed and may be offered based on facility staffing needs, performance, and mutual agreement between the LPN and facility. Our Inpatient Unit on the Mitchell Campus in Cheektowaga, NY, provides a relaxing home-like setting for select patients and their families. Our unique unit is only offered to patients whose pain or symptoms cannot be managed at home or are approaching their final stage. Hospice patients can also be admitted to the unit for short-term respite care. A Home Health Aide assists in providing quality patient care by performing assigned duties, health care tasks, and specified patient care activities according to the policies and procedures of Hospice Buffalo, Inc. and under the supervision of a licensed practical nurse and/or registered nurse. Assists in establishing and maintaining good interpersonal relationships with hospice patients, families, health care personnel, and the entire hospice staff and volunteers. Essential Duties and Responsibilities for a Home Health Aide (HHA): Assists with appropriate hygiene and grooming inclusive of bathing, hair shampoo, nail & skin care; oral hygiene; toileting/assistance with elimination; and weights. Assists with health related tasks and personal care procedures. Assists with activities of daily living and instrumental activities of daily living (i.e.) answering phone and doing laundry. Meets the patient's general nutritional needs through meal planning, shopping, meal preparation and clean up and feeding the patient if necessary. Assists in the meal preparation for individuals with special dietary problems under the direction of a professional staff member. Understands adequate nutrition and fluid intake. Demonstrates effective observation and reporting skills. Maintains high level of confidentiality in compliance with HIPPA law and policy. Demonstrates appropriate hand hygiene; waste disposal; use of Personal Protective Equipment; Bag (supply/tote) technique; and knowledge of Standard Precautions, inclusive of basic infection control procedures. Appropriately explains duties and functions of the Interdisciplinary Team including the role of the Home Health Aide. Documents to agency standards. Displays the ability to establish priorities and organize work and time appropriately. Anticipates problems and modifies plans and procedures to provide solutions. Elicited no founded patient/family complaints, staff complaints or unusual occurrences (i.e. reportable incidents). Demonstrates ability to communicate with patients and families in a supportive manner that is understandable and culturally appropriate. Maintains a clean, safe environment; provides assistance with safe transfer techniques, positioning and ambulation. Demonstrates normal range of motion.
