A Home Health Aide assists in providing quality patient care by performing assigned duties, health care tasks, and specified patient care activities according to the policies and procedures of Hospice Buffalo, Inc. and under the supervision of a licensed practical nurse and/or registered nurse. Assists in establishing and maintaining good interpersonal relationships with hospice patients, families, health care personnel, and the entire hospice staff and volunteers. Essential Duties and Responsibilities for a Home Health Aide (HHA): Assists with appropriate hygiene and grooming inclusive of bathing, hair shampoo, nail & skin care; oral hygiene; toileting/assistance with elimination; and weights. Assists with health related tasks and personal care procedures. Assists with activities of daily living and instrumental activities of daily living (i.e.) answering phone and doing laundry. Meets the patient's general nutritional needs through meal planning, shopping, meal preparation and clean up and feeding the patient if necessary. Assists in the meal preparation for individuals with special dietary problems under the direction of a professional staff member. Understands adequate nutrition and fluid intake. Demonstrates effective observation and reporting skills. Maintains high level of confidentiality in compliance with HIPPA law and policy. Demonstrates appropriate hand hygiene; waste disposal; use of Personal Protective Equipment; Bag (supply/tote) technique; and knowledge of Standard Precautions, inclusive of basic infection control procedures. Nurse Practitioner – NP – Infusion Center– Days We are seeking an experienced Family Nurse Practitioner (FNP) with oncology experience and pediatric experience (or a strong comfort level caring for pediatric patients) to provide patient-centered care across our Same-Day Access and Infusion Center. This role supports the full spectrum of infusion services, including both benign and oncology patients in adult and pediatric populations , and involves acute symptom management, assessment and treatment of therapy-related complications, care coordination, and close collaboration with a multidisciplinary care team.
