JOB SUMMARY: The Clinical Assessor / Coordinator, LPN is responsible for reviewing sub-acute care and long-term care referrals received from surrounding areas hospitals/facilities. The Clinical Assessor / Coordinator, LPN identifies actual and potential mental and physical health problems and accurately determines the necessary classification and regimes to assist with the appropriate placement of a potential admission to the Gurwin Jewish Health Care System. HOURS: Monday through Friday 9:00am-5:00pm EDUCATION AND EXPERIENCE REQUIREMENTS Licensed to practice as LPN in New York State. Minimum of one year experience working in a hospital, long term care or certified home health agency (CHHA) DUTIES : Demonstrates knowledge and the capacity to accurately interpret the PRI/RUG Classification System. Properly manage referral case load and respond in a timely manner via Navi/Allscripts and eFax. Accurately assesses the classification and patient disposition upon screening. Ability to coordinate referral via EMR including verification of benefits Ability to coordinate the bed board/census when needed. Ability to assess potential patients at partnering hospitals. Addresses /accommodates the age-specific physical, psychosocial and emotional changes in residents/patients when planning programs and/or delivering care. Maintains updated knowledge and understanding of the comprehensive services available for new admissions. Discusses/interprets patient's potential and present status with appropriate disciplines. Demonstrates the ability to establish and maintain a professional rapport with members of the health care team in facilities and referring hospitals. Demonstrates a high level of communications skills and interviewing skills with patients, families, and all disciplines. Communicates clearly and comprehensible to residents, patients, and family members, understanding changes in cognition an allowing sufficient time for the processing of information.
