Responsibilities We are actively hiring for an Registered Nurse (RN) PreOp PACU. Promote the mission, vision, and values of SCA . Assess patient’s preoperative and postoperative status at intervals according to facility or health care organization policies and procedures to include the patient’s cardiovascular system, respiratory system and airway management needs, neurological system, hemodynamic stability, pain and comfort needs, and mental status. Interpret information accurately from various noninvasive and invasive monitors. Describe the underlying pathophysiology of commonly encountered disease processes. Describe the physiology associated with various anesthesia techniques and surgical or procedural interventions. Apply the relevant scoring system/discharge criteria for patient transfer ( e.g. , Aldrete Score, Modified Aldrete Score [ i.e. , PARSAP], Post Anesthesia Discharge Scoring System [PADSS], White) score accurately as defined by organizational policy. Develop an individualized perianesthesia plan of care. Implement the plan of care including the following interventions: Preparing for and managing patient recovery from anesthesia – general. Preparing for and managing patient recovery from anesthesia – monitored anesthesia care. Preparing for and managing patient recovery from anesthesia – regional. Preparing for and managing patient recovery from anesthesia – local. Preparing for and protecting patient from injury caused by positioning, thermal sources, and extraneous objects. C ommunicating the patient’s current status throughout the perianesthesia continuum of care. P roviding acute pain and comfort management as needed. A dministering medications safely and correctly by using the 8 -rights of safe medication administration. P erforming interventions to maintain the integrity of the patient’s wound and tissue perfusion at or above baseline levels . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems, and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect patient from infection. M anaging patient’s other co-morbid conditions as applicable. M anaging patient’s fluid as prescribed. A ssessing knowledge level of patient or designated support person and providing education regarding the expected psychosocial response, nutritional management, medication management, pain and comfort management, wound management, and expected responses to the operative or invasive procedure; I nvolving patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice; and P roviding consistent and comparable care regardless of the setting. Evaluate the patient’s progress toward attaining outcomes. Transfer care as needed. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions. Document nursing care completely, accurately, and legibly. Collaborate effectively with other disciplines as needed. Participate in quality review and performance improvement projects. Participate in the performance appraisal process. Use p roblem-solving and conflict resolution skills to foster effective work relationships with team members. Maintain required competencies. Pursue professional growth. Act as a patient advocate and maintain privacy and confidentiality of individuals and health information. Comply with all facility policies and procedures when providing patient care . Actively participate in team meetings, trainings, and daily safety huddles . Demonstrate fiscal responsibility. Perform all other duties as assigned . Qualifications Graduate of an Accredited School of Nursing. 1-2 year of Emergency Room Experience Required. One year of Pre Operative or PACU experience required Current state licensure to practice within good standings. Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 days of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advanced Life Support (PALS) course is required when pediatric care is included in the facility scope services within 90 days of employment or documentation of current PALS certification. We are seeking a dedicated Licensed Practical Nurse (LPN) for our 7:00 PM–7:00 AM shift to provide high-quality nursing care to our residents while supporting and guiding the nursing assistants assigned to the overnight shift. This role is ideal for an LPN who is dependable, confident working during overnight hours, and committed to maintaining a safe, calm, and resident-centered environment. As an LPN, you will play an important role in ensuring our residents receive safe, compassionate, and consistent care in accordance with physician orders, facility policies, and applicable state and federal regulations. What You’ll Do Provide direct nursing care to residents throughout the 7:00 PM–7:00 AM shift. Oversee and support nursing assistants assigned to your unit or shift. Provide leadership, direction, and support to nursing staff while coordinating resident care needs throughout the night. Administer medications and treatments as ordered by physicians. Monitor residents for changes in condition and respond promptly to concerns or emergencies. Complete accurate and timely nursing documentation, including progress notes, physician orders, admissions, transfers, discharges, and incident reports. Receive and transcribe physician orders and communicate changes in resident condition as appropriate. Perform nursing procedures within the LPN scope of practice, including catheter care, tube feedings, dressing changes, ostomy care, specimen collection, range-of-motion exercises, and other ordered treatments. Provide IV and IV-site care as permitted by state regulations, licensure, training, and facility policy. Receive report at the beginning of the shift and provide a thorough report to the incoming day-shift team to ensure continuity of care. Communicate effectively with residents, families, physicians, nursing leadership, and other members of the interdisciplinary team. Help maintain a quiet, safe, respectful, and comfortable overnight environment for residents. Ensure nursing staff follow established safety procedures and infection-control practices. Protect resident privacy and maintain confidentiality of all resident and health information. Report incidents, concerns, or suspected inappropriate disclosure of resident information in accordance with facility policy. Perform other duties as assigned. What We’re Looking For Current, unencumbered Licensed Practical Nurse (LPN) license in the applicable state. Current CPR certification. Strong clinical judgment and the ability to work independently when needed. Ability to communicate clearly and professionally with residents, families, and team members. Ability to provide leadership and support to nursing assistants throughout the overnight shift. Strong organizational and documentation skills. Knowledge of nursing practices, documentation requirements, and regulations applicable to senior living or long-term care. Long-term care, skilled nursing, assisted living, or senior living experience is preferred, but not required. A dependable, team-oriented attitude with a genuine commitment to excellent resident care. Physical Requirements This position requires the ability to move throughout the community during the shift and perform the physical functions associated with resident care. The employee must be able to safely push, pull, move, and/or lift at least 25 pounds and may be required to assist residents during emergency situations. Registered Nurse (RN) SUMMARY: The Wound Care / Infection Control Nurse is responsible for providing primary skin care to residents under the medical direction and supervisor of the residents’ attending physician, the Director of Nursing, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. ESSENTIAL DUTIES AND RESPONSIBILITIES: Confer with the Director of Nursing and/or other licensed nursing personnel regarding the dermatologic disorders of residents in the facility Examine the resident and his/her records and charts, and discriminate between normal and abnormal findings, in order to recognize when to refer the resident to a physician for evaluation, supervision, or directions Provide assessment and diagnostic services to residents. Perform an assessment evaluation using techniques including observation, inspection, and palpation Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff Identify, manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic and viral skin infections, scaling popular diseases, and benign tumors Assist in resident rehabilitation activities. Where require, perform range of motion, assist in ambulation, provide emotional support. Monitor the results of physical therapy activities. Provide preventative health care services. Make recommendations to the resident concerning nutrition, prevention of common skin disorders, etc., Make written and oral reports/recommendations to the attending physician, Medical Director, or the DON concerning the status and care of the residents Implement and maintain established policies and procedures relative to skRN/LPNin care treatments. Interpret these to the physician, resident family members, and public as appropriate Performs other duties as assigned QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. LPN/RN with current unencumbered state licensure. Wound Certification from accredited institution preferred. Must have current CPR certification. The Medical Assistant will work under the direct supervision of a Registered Nurse (RN), Advanced Practice Nurse (APN), or Physician, assisting with a range of clinical and administrative tasks such as administering vaccines, conducting EKGs, recording vital signs, maintaining accurate patient records, and collaborating with the healthcare team to enhance patient care. Required Qualifications: A completed High School diploma or equivalent from an accredited body A current certificate of cardiopulmonary resuscitation training Must have phlebotomy experience Experience working in a community health setting Completion of a certified/registered program for medical assistants that is recognized by the American Heart Association or American Red Cross Graduate of a certified program for Medical Assistants (CMA credential) Preferred Qualifications: Considerable skill in communicating verbally, in determining priorities and in organizing work; ability to maintain an organized, alphabetized filing system General working knowledge of PC.
