Provide education, remediation, and assessment of skills competencies as directed by the DON/Nurse Manager. Monitor documentation for adherence to rules and regulations. Ensure environment of care requirements are met (ie: quality logs, infection control practices, etc) Utilizes the nursing process: assessing, planning, and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities. Identifies expected outcomes individualized to the client to include discharge planning; prescribes interventions to attain expected outcomes; evaluates the effectiveness of interventions in relation to expected outcomes; conducts ongoing assessment of clients from admission to discharge; initiates medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patient's conditions. Provides individual patient teaching on all identified learning needs; facilitates in Family/Significant other education. Demonstrates knowledge of psychiatric disorders and appropriate nursing interventions; implements nursing interventions identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Provides structure and maintains a therapeutic milieu in collaboration with the patient and other health care providers. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. Performs other duties and projects as assigned.

QUALIFICATIONS: Currently certified in New Jersey as a Certified Nursing Assistant. Ability to read, write and comprehend English. Long-term care experience preferred. POSITION SUMMARY: Under the supervision of a licensed RN or LPN performs nursing services based on the facility’s policies and procedures as regulated by the State of NJ, DOH and Federal regulations. Provides assistance to residents with daily routines and other resident activities as needed. PART I: JOB RESPONSIBILITIES AND STANDARDS 1. Follows the plan of care. Attends IDCP meeting as assigned. 2. Demonstrates proper body mechanics. 3. Provides personal care to residents/guests (bathing, cleaning finger nails, shaving and perineal care, etc.) and assures that resident/guest dresses appropriately. 4. Follows infection control procedures (i.e. hand washing/sorting of linens) 5. Performs routine /special skin care as indicated. Reports skin alterations immediately to nurse. 6. Provides privacy for residents and adheres to HIPAA standards. 7. Inquires about residents/guests comfort/discomfort level on a scale of 0-10. 8. Provides restorative care as indicated. 9.
