Description Full-time 40 hours a week Position Summary: Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Qualifications: Education/ Experience: Certification per American Association of Medical Assistants preferred (AHA) , LPN License, or LNA License. Other Requirements: Proficient computer skills required. Experience working with an electronic health record preferred. Requirements Essential Functions Of The Position: Treats patients and their families with respect and dignity. Facilitates patient flow throughout the encounter. Completes chart prep, driven by visit type and chief complaint including reviewing the medical chart for current information regarding the patient’s medications, test results, hospital records, operative reports, notes from consulting specialists, etc. Any missing results or other information should be requested and obtained. Maintains and updates clinical information including identifying and documenting active problems, completing/resolving historical problems, updating social and family history, reviewing and updating allergy list, reconciling patient’s current medication list, and determining if the patient needs refills after documenting any reported side effects of medications. Records chief complaint for visit, as obtained through patient interview. Performs patient intake including obtaining vital signs, contributing to the assessment of the patient’s health status, and recording results in the EMR. Maintains and updates preventative health screening documentation. Identifies psychosocial needs of patients utilizing the depression screening tool and communicates concerns to the patient’s medical provider. Utilizes nursing standards of care including assessment, plan, implementation and evaluation. Communicates the patient’s condition to the medical provider including any significant changes noted during patient assessment. Performs point-of-care testing per established policies and procedures. Assists medical provider with procedures. Maintains aseptic technique during treatments and special procedures. Reports results of laboratory and other tests to the patient when directed by the medical provider. Utilizes available communication methods, which may include EMR and telephone, to convey patient-related communication as necessary. Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations. Stocks patient rooms with necessary supplies. Maintains current knowledge of medications and administration of injections. Understands and applies the Six Rights to Medication Administration. Works in a multi-disciplinary team setting and seeks guidance from providers, Registered Nurses and Licensed Practical Nurses. Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage. Assists in the resolution of complaints, requests and inquiries from patients. Maintains a cooperative professional working relationship with staff including but not limited to medical staff, and other departments, as well as outside agencies, community members and our patients. Completes annual education, certification and/or license requirements. Adheres to regulatory requirements. Attends/completes all mandatory in-services and trainings.
