As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Case Manager Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing. The Nurse Case Manager will work under the direction of the Director of Nursing.

Key Responsibilities: Greet patients and escort them to the examination rooms; assist patients with the completion of forms as necessary Become familiar with clinic computer hardware and software and use according to company policies Schedule appointments and accommodate patient appointment needs, such as ordering transportation, rescheduling, etc. Perform clerical work as needed, i.e., copying, filing, faxing, etc. Comply with patient needs at all times and ensure all questions are answered Maintain procedure rooms by ensuring that they are neat and ready for use at all times Assist doctors during Endovenous Laser Therapy procedures in accordance with instructions and individual doctor preferences Prepare patients before procedures and clean up after Ensure patient receipt of post-procedure instructions and how to obtain medication if needed Monitor supply levels and replace as needed Apply knowledge of sterile techniques and OSHA regulations Prepare IV solution Train new staff as needed Assist ultrasound staff as needed Transport supplies or equipment as needed Performs additional duties as assigned High School Diploma or equivalent required Fluent in Spanish required Medical Assistant Certification - highly preferred 1-2 years of experience as a medical assistant Certification in Basic Life Support (BLS) Strong communication and interpersonal skills Ability to work well in a fast-paced team environment Health insurance (medical, dental, vision) Retirement Plan Paid time off (PTO) (vacation, sick) INDM2
