I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · provide Nurse Case Manager with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making referrals for home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. · reports to DON as patient condition changes, changes in the patient condition that require change in level of care. III. Orientation and TRAining Tender Care Hospice will provide orientation and training, as necessary, to assist the Case Manager Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Case Manager Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Summary The Licensed Practical or Vocation Nurse (LPN/LVN) is responsible for all departmental functions aligned with the mission, vision, and goals of PM Pediatrics. The LPN/LVN role is an opportunity for a pediatric LPN/LVN to apply their skills in a pediatric urgent care setting and assist with primary patient care based on individual patient needs within their scope of practice under the physician’s direct supervision. This position maintains effective working relationships with all customers, including patients/families, physicians, employees, volunteers, and vendors. Description Job Responsibilities: Provider clinical and administrative support for providers, patients, and office operations Obtain medical history, patient vital signs, intake, and prioritize patient care Perform procedures (venipuncture, IV insertion, urinary straight catheterization), administer medications (oral, rectal, IM, and IV), and assist the physician with total patient care within the scope of practice Remaining knowledgeable in pediatric pharmacology Be an integral part of the patient care team and assist with patient care within the scope of practice Perform monthly quality control checks in the office Assist in performing quality control testing in the lab Ability to assist the physician with patient care, positioning, and comfort during the x-ray process Keep exam rooms stocked with adequate medical supplies, maintain instruments, and proper sterilization tools and equipment as required Practice standard safety practices and precautions Helps patients in distress by responding to emergencies and promptly notifying physician Establish effective rapport with clients, families, other employees, and physicians Maintains operations by following policies and procedures; reporting needed changes to management team Contributes to team effort by accomplishing related results as needed Maintains safe, secure, and healthy work environment by following and enforcing standards and procedures, complying with legal regulations Maintains patient confidence and protects operations by keeping patient care information confidential in compliance with HIPAA regulations Update knowledge by participating in continuing education requirements and opportunities; regarding professional publications; maintaining personal networks; participating in professional organizations Enhances health care practice reputation by accepting ownership for accomplishing new and challenging requests; exploring opportunities to add value to job accomplishments Assisting patients with complaints and taking an active role in service recovery when needed Assist physician with a focus on efficiency metrics to improve urgent care flow and throughput (Arrival to Room, Arrival to Medication Administration, Disposition to Discharge, Discharge Length of Stay) Qualifications Pediatric experience is required; Pediatric ER, Pediatric hospital unit, or Pediatrics Urgent Care Experience preferred Active state nursing license or active compact state licensure from the state you reside Prior experience and competency with nursing procedures (not limited to IV placement, urinary straight catheterization, and medication administration) Current Basic Life Support (BLS) is required, Pediatric Advanced Life Support (PALS) recommended Knowledge of medical practice and terminology, clinic’s processes, and procedures Knowledge of medical equipment and instruments Skill in rapid assessment of the critical presentation of the pediatric population Ability to use sound reasoning and judgment and act calm in emergencies Ability to effectively communicate both verbally and nonverbally Possess the following qualifications: Energetic, Highly Motivated, Customer Service Focus, Positive Attitude, Confidentiality, Teamwork, and Professionalism Compensation: $30.50 - $35.50/hour The salary/rate range listed here has been provided to comply with local regulations and represents a potential base salary/rate for this role.
