Description The Clinical Oncology Nurse Navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge, who provides individualized assistance to the patients, families, and caregivers through informed decision making, timely access to quality health and psychosocial care, and coordination of appropriate follow-up visits throughout the cancer continuum. This individual helps overcome healthcare system barriers, influence the systems and behaviors, and address the gaps of care through research and evidence-based practices guided by regulatory committees and professional organizations. The Clinical Oncology Nurse Navigator supports the organizational strategies to deliver high-quality care to the population served through community needs assessment, navigation program development, marketing and outreach, and ONN specific-outcome metrics (i.e., care coordination, guidance, education, and advocacy) established by the organization. The Clinical Oncology Nurse Navigator initiates, develops, and implements mechanisms for effective collaboration with the interdisciplinary team, that includes (but not limited to), new patient coordinators (NPC), clinical oncology associates, clinical coordinators (RNs), clinical infusion nurses, advanced practice providers (NPs & PAs), physicians, social workers, and nursing/administrative leadership for the attainment of the highest standards of patient care. Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients and more. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information. Performs other related duties as assigned. Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. 1+ year of experience in Nursing or relevant area.
