The Licensed Practical Nurse's primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. Those services are performed under the supervision of a registered nurse but utilize all nursing skills to make safe, appropriate and independent decisions within the scope of the LPN nursing practice. Essential Functions: Delivers quality nursing care to assigned group of patients by collecting data and identifying deviations from normal; assists with reassessing patients according to assignments, need and unit protocol and/or hospital policy under the direction of the registered nurse. Accurately and thoroughly documents all findings in the patient record and identifies expected outcomes based on assessment data; effectively communicates pertinent assessment findings with other health care members. Implements nursing care interventions identified in the individualized multidisciplinary treatment plan using accepted standards of practice in collaboration with other health care providers; performs nursing procedures/treatments as prescribed by physician/LIP. Conducts specified bedside laboratory tests, wound care, and provides for comfort and assistance in the performance of ADLs; Promotes the patient's ability to cope, adapt and/or solve situations related to illness and/or stressors. Provides supportive, age appropriate communication and culturally acceptable interventions and documents special needs. Provides for the safe delivery of medications in accordance with the facility policies and physician orders; administers medications utilizing established safety procedures / standards. Ensures patients are effectively assessed prior to and after medication administration. Accurately communicates/documents medication doses prescribed, and provides patient education on medication administered. Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care; transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines. Utilizes nursing knowledge to successfully meet the needs of the facility, patient population and staff members. Utilizes nursing resources to enhance evidence-based practice. Guides, educates, and provides leadership as a preceptor to new staff. Performs other duties and projects as assigned.

Licensed Practical Nurses must adhere to all relevant regulations and guidelines to ensure patient safety and confidentiality. They are expected to maintain a high level of professionalism and compassion during all interactions. We’re Looking For: Licensure or Certifications Licensed Practical Nurse (LPN) license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days unless department leader has determined it is not required. Degrees: LPN: Advanced training beyond High School that includes the completion of an accredited or approved program in Practical Nursing What You’ll Do: As a Licensed Practical Nurse , you’ll play a vital role in the care team – supporting providers, connecting with patients, and helping ensure a seamless, compassionate experience every day. As a key member of the ambulatory care team, you will: Welcome patients and support their transition into care. Take and record vital signs, medical histories, and health screenings. Prepare patients for exams and assist providers during procedures. Administer medications and vaccines as directed. Perform lab-related tasks including specimen collection and point-of-care testing. Document care accurately in the electronic health record. Partner with physicians, nurses, and teammates to deliver high-quality, coordinated care. Physical Requirements and Working Conditions Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present. Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions. Maneuver foot pedals on carts or machines. Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed. May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary. Must be able to respond quickly to changes in patient and/or unit conditions. Physical Abilities Testing may be required. Proficient computer skills including keyboarding, navigation within a windows operating system, and use of electronic mail with exposure to electronic medical records systems. The Medical Assistant/EMT Position Responsibilities Include Room patients and complete vital signs and other pertinent physical measurements. Administer medication and clinical procedures according to authorized policy and documented according to HealthPartners policy. This may include DISCUS or AIMS assessment. Review charts for the presence of pertinent information relevant to the patient’s appointment. This may include Prepared Practice work as developed by the team. Provide and document any needed and requested health educational materials to the patient. Handle patient and their records in a private and confidential manner per HealthPartners policy. Seek medical and/or nursing assistance when encountering an acutely ill patient or a patient with a changing condition. Prepare exam rooms and treatment rooms and set up for procedures. Assist provider as needed. Prepare the patient for medical interventions. Prepare charts and records for appointments as indicated by the type of clinic visit according to established practices. Check MSRs for completion and route appropriately. Prepare lab, x-ray and miscellaneous forms as requested Process drug refills according to clinic protocols Monitor station logs as assigned. Facilitate timely and accurate flow of communication within the clinic/location. Accurately receive and relay messages and test results to the appropriate person/location in a manner consistent with clinic practices and/or standards. Gather all necessary and appropriate information from a caller and relay information to the appropriate provider for necessary follow-up. Inform reception desk of provider’s schedule status and assist in coordination of urgent and non-urgent provider schedule changes to ensure patient and provider needs are met. Perform patient correspondence when requested and assist patients in completing forms when appropriate. Prepare and sterilize instruments and equipment according to procedure and OSHA standards. Complete scheduled cleaning requirements for designated work areas. Document completion of work to comply with infection control and OSHA standards Implement other infection control procedures as needed and adhere to protective clothing requirements. Ensure clinic supplies are adequately stocked and well organized. Process and deliver department inventory requests in a timely manner. Routinely check expiration dates and remove or replace outdated supplies. Maintain prescription sample supply cabinet as per clinic protocol Ensure all equipment is properly maintained and adequate records are kept. Ensure inventory of instruments and equipment is current. Ensure instruments and equipment are calibrated as needed and maintained in proper working order. The Medical Assistant, EMT and LPN position requires the ability to sit or stand for prolonged periods of time. Must possess speech, hearing, and sight skills to perform the functions of the position. Manual dexterity required. Must be able to speak, read and write English proficiently.
