Responsibilities Works in a variety of clinical settings providing medical and counseling services as a member of the multi-disciplinary treatment team. Provide client services for medical issues. Monitors and documents clients’ medical status, signs and symptoms of withdrawal and reports to supervisor or medical director. Works with other medical staff for continuity of clients’ medical care. Communicates with clinical staff regarding clients’ medical concerns. Works directly with clients in providing medical services and interpreting medical information. Provides direct client care of medical assessment, implementation of medical orders, communicable and infectious disease reporting in accordance with the law. Provides public health and screening protocols. Provides and documents medical consultation, assessment, examination, and treatment planning and review consistency with agency policy and requirements Qualifications Must be a licensed practical nurse currently registered by the New York State Education Department. Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. The LPN will also provide patient care by administering medications, processing patient phone calls per practice protocols, instructing patients regarding health care and treatments, and providing health care provider with information pertaining to patients' health status. May be required to float to other UFHP practices, as needed, for staffing. Qualifications Minimum Education and Experience Requirements: Licensed LPN eligible to practice in the State of Florida. One year of ambulatory care, triage, or hospital experience preferred. Person should display initiative and have the ability to multitask. Outstanding patient/customer service skills, verbal clarity, team focus, and problem-solving skills are required for the ideal candidate. Motor Vehicle Operator Designation: Will not operate vehicles for an assigned business purpose Licensure/Certification/Registration: Current Licensed LPN eligible to practice in the State of Florida. Florida is part of the Nurse Licensure Compact (NLC) Act. AHA or Red Cross Basic Life Support (CPR) is required or must be obtained within first 90 days of employment. Must maintain current Basic Life Support (CPR). Must successfully complete medication test within 30 days of hire.

We are now hiring a skilled part time Licensed Practical Nurse in River Edge, NJ . The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
