Job Description Nemours Children’s Health is seeking Advanced Practice Providers to join our Rheumatology Team in Jacksonville, Florida! Our Rheumatology team works to diagnose and treat conditions that involve joints, muscles, skin and other areas. Our Physicians and Advanced Practice Providers are experts at treating a wide range of conditions that can negatively impact a child’s comfort and wellbeing. Key Responsibilities Provides care to pediatric patients as guided by education, clinical expertise and Scope of Practice Agreement, including ambulatory, inpatient (acute) care, surgical first assist and pre- and post-operative management Elicits comprehensive or focused/interval health history Performs complete or interim physical examinations as indicated by the nature of the encounter Orders, performs, and/or interprets diagnostic or screening tests, including laboratory, radiological or other tests within the APP'S scope of practice Plans interventions including medications, treatments and procedures, and modifies plans based on evaluation of patient response. This role will cover during after hour clinic, primarily evening hours. ESSENTIAL RESPONSIBILITIES Provides appropriate pediatric patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering lab and diagnostic tests, interpreting test results, and providing accurate medical record documentation. Establishes presumptive diagnosis as defined in the written agreement between the PA-C and the supervising physician(s). (20%) Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the PA-C knowledge base or scope of practice. (15%) Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the PA-C and the supervising physician(s).
