Location: Main Hospital Bldg - 2nd Floor PRIMARY PURPOSE The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The Oncology Nurse Navigator directs the coordination of care for patients with Hereditary Hemorrhagic Telangiectasia (HHT) and Lymphoma patients. As an Oncology Nurse Navigator (RN) for the HHT and Lymphoma Teams, you serve as clinical liaison and the single point of contact for patients, coordinating their care as they move from initial diagnosis, treatment, and survivorship. You help ensure timely delivery of care and eliminate potential barriers, identify resources, and serve to enhance patient care and the patient experience.
