The Diabetes educator RN provides diabetes self-management education/ training to individuals and groups according to the ADA SOC for Diabetes and Association of Diabetes Care and Education Specialists standards. The Registered Nurse is a member of the interdisciplinary team working in the out-patient diabetes education program. Responsibilities Conduct patient assessments and develop personalized educational plans for individuals with diabetes Provide comprehensive diabetes education to patients and their families, covering topics such as blood glucose monitoring, medication management, insulin administration, nutrition, and lifestyle modifications Teach patients how to use diabetes care technologies, including glucose meters, insulin pumps, and continuous glucose monitoring systems Collaborate with the healthcare team to develop and implement treatment plans, making recommendations for adjustments as needed.
