JOB DESCRIPTION " Position Summary The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of AtlantiCare LIFE Connection. The Transition of Care Nurse also is responsible for the coordination of care of all AtlantiCare LIFE Connection participants currently residing in an acute care setting. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation. Qualifications EDUCATION: Graduate of an accredited school of Nursing required. Bachelor Degree in Nursing preferred. LICENSE: Current Licensure as a Registered Nurse in the State of New Jersey. JOB DESCRIPTION " POSITION SUMMARY The Perioperative Team Leader is a member of the interdisciplinary team. The Team Leader is responsible to oversee any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. Cath Lab (cath lab holding, Electrophysiology (EP)) Critical care, emergency room, cath-lab or telemetry experience preferred; ACLS at time of hire or transfer Endoscopy 2 years medical/surgical experience preferred; Endoscopy, telemetry and critical care experience preferred. ACLS required within 6 months from date of hire or transfer PALS, CCRN, and CGRN preferred. Radiation Oncology (cancer center institute) Oncology Experience Preferred. OCN preferred. Medical Oncology Infusion Services (Cancer Center Institute) Oncology Experience Preferred ONS Chemotherapy and Biotherapy Competency Provider required within 6 mos. from date of hire or transfer. OCN preferred Structural Heart 2 years' experience in cardiology Able To Perform: Phlebotomy EKG IV insertion ACLS required at time of hire or transfer QUALIFICATIONS EDUCATION/LICENSE: Graduate of an accredited school of nursing; BSN preferred. Nurses hired without BSN must be enrolled in a BSN program within 1 year and completion within 3 years. Current Licensure as a Registered Nurse in the State of New Jersey or current multi state license required. BLS required. Valid NJ Driver's License is required for all employees in the Adult and Child Partial Programs. EXPERIENCE: Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
