Provides oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities. Ensures the timely start of patient care within recommended regulatory guidelines. Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate). Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness. Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician. Conducts regular OASIS reviews for all patients. Meets with all direct care team members on a regular basis providing guidance, coaching and communicating information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff. Communicates frequently with the Branch Director or Administrator on key performance metrics. Remains available for field work, as needed. What is required? Associate degree in a Nursing related field, Bachelor preferred 2 years of home care experience in Home Health or Hospice environment 1 year of supervisory experience required Current, unrestricted RN license valid for the state of work Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office Able and willing to travel up to 50% as needed to support business and patient needs. Department Overview At Children’s Minnesota PICU in St. Paul Campus, our interdisciplinary team of intensivists, pharmacists, respiratory therapists, nutritionists, social workers and case managers are dedicated to the care of children with life-changing critical diagnoses. We specialize in caring for children experiencing acute and chronic neurological and respiratory illnesses, recovery of extensive surgical interventions and severe infections, in addition to failure of one or more body organ systems. The PICU at Children’s St. Paul is a 12-bed unit for critically ill patients and their families. We care for patients along the continuum of acute recovery to provide for their changing physical and emotional needs as we prepare them to return home. Our patient population requires complex care, including hemodynamic monitoring, critical medication management, maintenance of systemic perfusion and management of organ failure. Our team balances a highly technical skill set with a family-centered approach. We rely on teamwork, with open and respectful communication, and promote an environment that encourages learning and professional growth. Position Summary Coordinates and provides family-centered nursing care to infants, children and adolescents, utilizing the nursing process. Accountable for professional practice including independent nursing functions and delegated medical functions performed in collaboration with other health team members. THIS IS AN MNA CONTRACT POSITION. Education Requirements: * Graduate of an accredited school of nursing. * Current Minnesota RN licensure. * Current BLS certification. * Based upon the unit in which you are applying, additional certification(s) may be required. Department specific required qualifications need to be obtained within 6 months of employment. * Current Pediatric Advanced Life Support (PALS) certification required or obtained during orientation.
