The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Job Description Nemours is seeking a full-time Registered Nurse to join our Nemours Children's Health team in Wilmington, DE. 3 West is a 24-bed medical surgical unit for patients 0-21 years of age. This unit is designated for orthopedic patients, patients in need of comprehensive rehabilitation and acute rehabilitation of both traumatic brain injury and spinal cord injury, ventilator-dependent patients, and patients with tracheotomies. Additional patient populations include neurosurgical and nonsurgical neurology, and infants transitioning from the Neonatal Intensive Care Unit. Neurology and neurosurgical patients are also cared for on 3 West. This unit was designed to provide single patient care rooms to allow for privacy and family presence during the hospital stay. Position is full-time, rotating shifts, 60 hrs in 2 wks, 12-hr shifts, 2/6 weekends, every other holiday. The primary role of the Registered Nurse is to implement the patient care delivery model for the patient and their family.
