We are seeking a Field Nurse Case Manager to cover the South Bay Area in CA, and surrounding areas . A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). These appointments would be to doctors’ offices, hospitals, and various other locations. There may be multiple appointments in 1 day. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. The RN (CN III) facilitates the smooth operation of the nursing unit, acts as a mentor, preceptor, and resource for the Clinical Nurse I and II, as well as other staff members, and provides advanced consultation to other units and hospital staff regarding complex critical care patient care problems. This position is represented by SEIU, Local 121RN, and is covered by the terms and conditions of the applicable collective bargaining agreement. Job Requirements Minimum Current California Registered Nurse license Current AHA BLS CPR certification Current AHA ACLS certification Three (3) to five (5) years current critical care experience Completion of critical care competencies within 6 months of employment.
