This role is focused on delivering high-quality clinical assessments, care planning, and ongoing support for residents through the New York State Uniform Assessment System (UAS-NY). As part of our interdisciplinary care team, the ideal candidate will be a compassionate, detail-oriented professional who is experienced in conducting in-person assessments, developing individualized care plans, and working closely with residents, families, and healthcare providers to promote resident well-being and regulatory compliance. Duties: Perform thorough in-person clinical evaluations for existing residents according to UAS standards, agency policies, and state laws. Create individualized service plans and determine eligibility for ongoing participation through detailed UAS assessments. Evaluate potential new residents by considering clinical, social, and environmental factors to establish appropriate levels of care ensuring safety and well-being. Gather and analyze clinical information from multiple sources to support patient care decisions based on clinical guidelines and professional judgment. Maintain clear and effective communication with residents, family members, healthcare providers, and other stakeholders to guarantee assessment accuracy. Apply a strong understanding of the challenges faced by vulnerable populations with chronic or complex bio-psychosocial needs. Ensure all assessments meet regulatory and organizational requirements. Requirements: Valid Registered Nurse (RN) license. UAS-NY Certification required Experience or familiarity with UAS-NY strongly preferred. Excellent communication, organizational, and interpersonal abilities. Self-motivated with the capability to work independently. Proficient computer skills and fluent English communication. Prior experience in MLTC or Managed Medicaid environments is a plus.

Quality, Safety and Experience Improvements 1. Responsible for VMS safety, quality idea boards on unit. 2. Accountable for the execution of daily and shift safety and clinical care huddles. 3. Rounds on existing and new patients, addressing issues or concerns of patients and/or families on the unit. 4. Responsible for the closure of Safety Intelligence reports and appropriate Corrective Action Plans in place. Responsible for reporting all Serious Reporting Events (SRE) to leadership and provide feedback to staff on results. 5. Links departmental goals to department of nursing/organizational goals and strategic plans. 6. Actively coordinates clinical quality improvement and safety efforts on the unit with defined medical center quality and safety goals. 7. Facilitates patient flow through timely admissions and discharges in collaboration with medical staff and case management to assure proper utilization of resources. 8. JOB DESCRIPTION Distinguishing Characteristics Graduate Nurses (T&RC) and Registered Nurses 1 (T&RC) perform similar nursing tasks and activities; coordinate and provide nursing care to individuals in State University of New York teaching and research center hospitals; administer treatments and nursing procedures within established guidelines and as prescribed by licensed health care providers; and consult on nursing matters with treatment teams and other disciplines. Levels in the series are primarily distinguished by scope and independence of operation, which is based on incumbents’ credentials (i.e., possession of a limited permit as a graduate nurse or eligible to enter examination for licensure as a registered professional nurse; or possession of a license and current registration as a registered professional nurse in New York State).
