Provide exceptional care by responding promptly to patient requests, proactively anticipating patient needs, and resolving them. Educate patients, families, and caregivers about the patients medical condition, treatment plan, medications, possible side effects, and follow-up measures, ensuring complete understanding by translating complex medical terminology. The Wound, Ostomy, Continence Nurse (WOC Nurse) will be assigned to perform and assist with assessments, interventions, and evaluations for patients with actual or potential wound, ostomy, and/or continence problems. The WOC nurse is responsible for providing direct and indirect patient care, education, and clinical consultation in the outpatient and inpatient settings. The WOC nurse will collaborate/coordinate care with overseeing providers, nurses, and other health care team members to assess patient needs and to establish and execute an appropriate plan of care to meeting the rehabilitation needs of the patient. The WOC nurse will communicate with providers, coworkers, other staff and patients/families as appropriate about changes in the patient’s clinical condition and is able to respond appropriately to changes in condition or response to treatment. Responsibilities include the systematic collection and assessment of data in collaboration with the multidisciplinary team to provide therapeutic care for the patient and/or family.
