Perform admission assessments for new admissions into contracted facilities and document information in the EMR within specified time frame. Plan the delivery of patient care in collaboration with other disciplines to provide comfort and symptom management. Perform daily assessment and coordinate the plan of care with inpatient staff for General Inpatients (GIP). Assess patient for safety risks, abuse and neglect. Assess and document pain using the appropriate pain scale and manage patient’s pain. Document changes in patient condition and effectiveness/timeliness of care/interventions. Evaluate, update and revise the plan of care to facilitate achievement of expected outcomes. Know action, usual dosage, indications and side effects of medications administered. Responsibilities: Patient Care: Provide direct patient care by assessing, planning, implementing, directing, and evaluating nursing interventions to ensure optimal outcomes. Collaborative Teamwork: Collaborate with physicians, fellow nurses, and other healthcare professionals to develop and implement individualized patient care plans. Function in one unit on a particular shift. Relate effectively with other shifts for continuity of care. Maintain satisfactory relations with other departments and nursing units. Documentation and Reporting: Maintain accurate and detailed medical records of patients' conditions, treatments, and progress. Report any changes or concerns promptly to the appropriate team members. Medication Administration: Administer medications and treatments as prescribed, ensuring adherence to hospital policies and protocols. Supervision: Participates in all phases of education, maintenance of records, and upgrading of policies, procedures, and skills of personnel.
