Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Overview Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN may work in a variety of settings in the Critical Care/Telemetry Complex and roles within Riverside Health System. Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Facilitates safe patient transitions to the next care setting as part of plan of care. Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents. Ability to delegate and prioritize tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Able to recognize and respond to emergency situations. Evaluates own needs for support and uses identified systems to meet need. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care.
