Overview Collaborates with the Emergency Department Staff and Clinical AssessmentTeam to provide direct care for Emergency Department psychiatric patients to include facilitating social and emotional needs, collaborating with Emergency Department RN and Physician on medication schedules and evaluating patient progress. Responsibilities Assesses condition on each assigned patient on admission and on an ongoing basis to include but not limited to the patient’s physical condition, psychological status, social status, spiritual and cultural needs, pain assessment/management, nutritional and functional status, developmental or age related factors, and discharge planning needs Plans, provides, and documents patient and/or significant other education based on physical, behavioral, psychological, developmental, and age-related factors, after identifying barriers to learning. Evaluates and regularly reevaluates patient care; coordinates care planning with patient, family, providers and other disciplines to achieve patient-centered outcomes Assigns and delegates nursing care to team members in accordance with the patient’s needs and the capabilities of the team members Applies safety principles when performing care Accurately documents observations, care provided and changes in care plan Provides accurate handoff reports and participates in unit based huddles Assists in performance improvement (PI) activities by serving on PI teams as warranted, assisting with PI measures and supporting and implementing changes necessary for improvement. Performs initial and annual competency per job class DCH Standards: Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5.
