Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload. Communication Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Encourages development of relationships with partners and referral sources. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary items and services are available. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Requirements Currently licensed as a Registered Nurse (RN) in the State of Arizona. Hospice or home health experience strongly preferred. Strong clinical judgment and ability to assess and prioritize patient needs. Excellent communication, organization, and problem-solving skills. Comfortable working independently with confidence and professionalism. CPR certification required. Must provide proof of valid Arizona driver’s license, reliable vehicle, and current auto insurance. Essential Functions: Coordinate individual clinical research projects in multiple disease conditions. This coordination for the patient includes reviewing medical history for eligibility, educating the patient to the study requirements, registering the participant to the project, and organizing patient visits. Patient visits requirements involve scheduling, reviewing treatment plans, assuring study treatment and supplies are available and documenting the encounter. This documentation must meet Good Clinical Practice standards of attributable, legible, contemporaneous, original, accurate, and complete. Lastly follow- up with the participant related to next visit and study requirements. Administering and dispensing investigational medication ordered for that patient by a licensed physician in the dosage prescribed by protocol. Retrieval of medication and assessment of compliance by patient to the protocol. Other Duties: Routine travel between Guthrie research sites may be required, as well as periodic travel for protocol training. Collaborate with director, manager, research team members and IRB coordinator in developing and revising process flow for research procedures. Responsibilities You will organize patient care by prioritizing, delegating, delivering and evaluating on an ongoing basis You will utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to hospital policies, procedures and protocols Collaborate with the nursing team to facilitate overall improvement in quality of care, patient experience and cost reduction Foster a learning environment for patients/families, nursing staff and members of the health care team Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds Requirements Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL) Current BLS Certification required ACLS or NALS or PALS or TNCC or NRP certification, as required by facility or department Certification in specialty area preferred One (1) year of experience in acute care setting preferred About Tacoma General Hospital MultiCare Tacoma General Hospital, in the heart of Tacoma, WA, is a 367-bed regional medical center delivering cutting-edge care. Pay and Benefit Expectations We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $48.15 - $90.83 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
