Key Responsibilities Assess patients for hospice eligibility and appropriateness of services Provide education to patients, families, and healthcare professionals regarding hospice philosophy, services, and benefits. Collaborate with hospital physicians and Hospice Medical Directors to confirm hospice eligibility. Coordinate and communicate key clinical information to ensure a smooth transition to the hospice admitting nurse. Work closely with hospital case managers, discharge planners, physicians, and other healthcare professionals. Serve as a trusted resource and advocate for patients and families exploring end-of-life care options. What you will need: A current license to practice as a Registered Nurse in the State of New York Associates Degree in Nursing, BSN preferred 1 year of recent acute care RN experience required Hospice, Palliative Care or RN Liaison experience a plus but not required.
