The Cleveland Clinic Neurological Intensive Care Unit is a state of the art 24 bed academic program led by 11 neurointensivists, 15 APPs, and 5-6 Neuro critical care fellows with additional resident support from Neurology, Neurosurgery, and Anesthesia. Patient management is multidisciplinary in conjunction with members of neurosurgery, neurology, vascular neurology, specially trained RNs, dedicated respiratory therapists and PharmD as well as others. Our multidisciplinary program dedicated to treating patients with acute neurological diseases, include acute ischemic stroke, ICH, SAH, subdural hematoma, status epilepticus, and meningitis, as well as many other neurological diseases and disorders. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. The NP or PA may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population. Diagnosis. Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions. Formulates Plan of Care. Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address complex acute and chronic health care needs. Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care. Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic, and surgical, needed to achieve expected outcomes. Manages further diagnostic tests through ordering, interpretation, performance, and supervision. Communication and Collaboration. Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the outpatient continuum of care. Documentation. Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries in medical record per specific patient unit or service, death summary) Documentation is timely, meets critical care compliance standards and captures patient acuity. * Paid one on one training and education provided. ​ * Empower, engage and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment.​ Primarily works in the clinic setting and conducts initial home visits with another staff member present​. * Position functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours​. PURPOSE AND SCOPE: Functions as part of the Home Therapies dialysis health care team. Ensures provision of quality home patient care in accordance with company policies, procedures and training. Supports the company’s commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction. Actively participates in process improvement activities that enhance the likelihood that patients will achieve the company’s Quality Enhancement Goals (QEP). PRINCIPAL DUTIES AND RESPONSIBILITIES: Staff Related: Provides safe and effective training and oversight of the delivery of home dialysis therapies to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the Corporation, state and federal agencies. Reports adverse patient events at home or in the clinic as well as equipment and technology related problems to the Clinical/Home Therapies management and physician including the correct documentation of such events. Coordinates the transfer of patients to the hemodialysis clinic when needed and appropriate. Laboratory Related: Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System. Follows company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results. General Duties: Maintains competency with all emergency operational procedures, and initiate CPR and emergency measures in the event of a cardiac and/or respiratory arrest when the patient is in the medical office. Administers medications as prescribed including following algorithms (as appropriate), and document appropriate medical justification if indicated and maintain records on controlled substances as required by law. Maintenance/Technical: Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone. Directs patients in the preventative maintenance of home therapies equipment, including water systems and analyses as appropriate. Coordinates with the company products division on the delivery, operation and maintenance of all home therapy dialysis equipment. Medical Records & Documentation: Admits all new patients and documents as needed. Documents all relevant data including physician orders, lab results, vital signs, and treatment parameters, and patient status and any treatment related data appropriately, and enters into Medical Information System. Participates in and ensures all company policies regarding patient admission, transfer, and discharge are appropriately implemented.
