Job Role Summary The Quality Review Registered Nurse (RN) position assists the Vice President of Long Term Care and other department personnel in the review and analysis of care and services provided by Health and Hospital Corporation (HHC) owned nursing homes and licensed residential facilities. Performs on site and office review of various clinical resident care services and nursing management operations at the long term care facilities and prepares related reports for the long term care department, individual facilities and the contract management company. Assists in the analysis of various aspects of nursing home operations using professional standards of practice and industry benchmarks and in the preparation of reports for the HHC Board of Directors’ Planning Committee. This position is responsible for promoting health and wellbeing of long term care residents of Health and Hospital Corporation Marion County nursing facilities by promoting preventive, therapeutic, and supportive care based on standards of practice and in compliance with all applicable regulatory requirements.Essential Duties Responsibility 1: 70% Performs quality review oversight visits to HHC owned nursing homes and retirement communities as assigned. May serve as a member of a quality review team or at times may visit assigned facilities as the sole representative from the HHC Long Term Care Department. Maintains a professional and supportive role when interacting with the nursing home and licensed residential facility staff providing helpful observations and suggestions. Performs various auditing/review activities during the site visits including but not limited to: medication administration observations, wound care and treatment administration, personal care services, meal service, clinical record reviews and review of nursing management records. At times may assist the other members of the quality review team to complete their assigned tasks. Other review tasks may include: resident or family interviews, review of quality assurance records, in-service training records, personnel files, incident report logs, professional facility consultant reports and other records as assigned. Must be knowledgeable and mindful of potential liability and other risk management issues and methods for taking appropriate preventative or corrective action. Participates in the exit conferences held with facility management and the quality reviewers and provides summary information regarding significant observations during the facility visit including specific resident and employee identification when possible. When requested, participates in taking notes at the exit conference for exit conference reports. Prepares a written site visit report following each facility visit in cooperation with other quality review team members who participated in the facility visit. Reviews report for accuracy, and clarity. Distributes reports in a confidential manner to all parties and maintains HHC records. Assures the retention and confidentiality of all data materials from facility visits and forwards to the department Executive Assistant for proper storage/retention. Responsibility 2: 30% Participates in the compilation and completion of quarterly and other reports prepared for the HHC Board of Directors’ Planning Committee. May attend HHC Planning Committee meetings as assigned to represent the Long Term Care Department or to present a special report. Assists in the completion and follow up of any requests or directives resulting form the Planning Committee meetings as assigned. Accepts phone calls from residents, family members and interested parties in the absence of other Long Term Care Department staff or as assigned. Receives concerns or information in a manner that reflects good customer service practices and prepares detailed written information for the Vice President’s review and processing. Attends community and professional association functions and meetings representing HHC Long Term Care division as requested. Participates in the development of educational materials and staff training programs relative to long term care. Assists in development of departmental policies, procedures and forms for use in the departmental functions. In the absence of the Executive Assistant, makes travel arrangements/reservations for department staff conducting out of town facility site visits. Must be able to communicate effectively in verbal and written communications and able to adhere to deadlines and interruptions and frequent assignment changes, etc. Must have good customer service skills for interacting with facility residents and personnel on site visits as well as the general public. General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility’s information system. • Perform only those nursing care procedures that you have been trained to do. • Ensure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident’s personal possessions as instructed. • Store resident’s clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents’ to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. • Ensure that residents who are unable to call for help are checked frequently. • Answer resident calls promptly. • Check residents routinely to ensure that their personal care needs are being met. • Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). • Record the resident’s food/fluid intake. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. • Keep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). • Participate in and receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. Record and report data as instructed • Perform special treatments as instructed. • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. • Use only the equipment you have been trained to use. • Report defective equipment to the Nurse Supervisor/Charge Nurse • Maintain the confidentiality of all resident care information including protected health information. Job Requirements POSITION SUMMARY The Nurse Manager is fully engaged in providing Product Excellence, Delight, Access, Ease and Convenience, and Customer Cost by planning, managing, and assessing all unit functions to include evaluation of patient care on a twenty-four (24) hour basis on the Nursing department of oversight. PRIMARY ACCOUNTABILITIES Manages and assumes responsibility for twenty-four seven (24/7) operations and activities of unit(s) of oversight. Facilitates and supports high quality patient outcomes; ensures compliance with The Joint Commission (TJC), Health First’s policies and procedures, and regulatory agency standards with quality indicators and core measures. Performs clinical skills within the scope of practice of a registered nurse in the State of Florida for which competencies have been validated. Oversees and monitors the unit metrics on patient experience as evidenced by positive patient satisfaction scores. Ensures consistent performance of purposeful hourly or team rounding, bedside shift report and no pass zones. Assures all equipment on the unit is operable and safe, while monitoring and ordering unit supplies and equipment with adequate inventory of supplies for unit needs. Prepares and adjusts 24-hour staffing plan based on unit census and acuity on a daily basis. Ensures schedules are completed on time and in compliance with approved staffing plan.
