Perform: Medical history and physical examinations Review of systems Vital signs collection Medication review and reconciliation Possible preventive health screenings Hemoglobin A1C Spirometry KED (Kidney Health Evaluation) DRE (Diabetic Retinal Exam) FIT Patient Education & Engagement Deliver preventive health education and counseling opportunities. Communicate findings used to generate post visit summaries for the primary care provider. Identify and help close quality care gaps (e.g., screenings, labs, follow-ups). Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience. Compliance & Professional Standards Adhere to HIPAA regulations and safeguard protected health information (PHI). Maintain accurate, timely, and compliant EMR documentation. We are looking for a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the Oak Ridge, TN/Knoxville, TN area to administer patient care in accordance with a physician established care plan. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patient needs range from weekly hours to 24-hour care. This allows for multiple scheduling options, from short visits on a weekly basis to full 8 and 12 hour shifts or a blend of both. Patients range from high acuity to those who only need basic medical management. Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come. You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. Position Overview The Patient Care Associate provides basic nursing care (i.e. personal and restorative care) to patients according to predefined guidelines; a plan of care established by a Registered Nurse or LPN; and under the direct supervision of the Registered Nurse or LPN. The plan of care will utilize the CDC Guidelines for Infection Control as well as the Singing River Health System Departments of Nursing Plan for Provision of Care, Treatment, and Services. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees.
