Provides supervision to Hospice Aides/Homemakers. Education a. Graduate of an accredited registered nursing program. b. Bachelor’s degree in Nursing preferred. c. Certified Hospice and Palliative Nurse (CHPN) certification preferred. 2. Experience a. Has minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience. b. Experience with pain and symptom management interventions preferred. c. Ability to work within an interdisciplinary setting. d. Understanding of Hospice philosophy, principles of death/dying. 3. The neonatal intensive care unit (NICU) at UMass Memorial Medical Center – Children’s Medical Center is the region’s only Level III NICU, caring for extremely premature babies and newborns with complex medical needs. A Level III NICU is equipped to treat babies born as early as 22 weeks' gestation. As a regional referral center, we also receive babies from other birthing centers who need the high level of care our team provides. I. Major Responsibilities: Clinical Practice, Patient Flow, Patient Experience: 1. Leads with high reliability principles framework with goal of zero harm. 2. Quality, Safety and Experience Improvements 1. Responsible for VMS safety, quality idea boards on unit. 2. Accountable for the execution of daily and shift safety and clinical care huddles. 3. Rounds on existing and new patients, addressing issues or concerns of patients and/or families on the unit. 4. Responsible for the closure of Safety Intelligence reports and appropriate Corrective Action Plans in place. Responsible for reporting all Serious Reporting Events (SRE) to leadership and provide feedback to staff on results. 5. Links departmental goals to department of nursing/organizational goals and strategic plans. 6. Actively coordinates clinical quality improvement and safety efforts on the unit with defined medical center quality and safety goals. 7. Facilitates patient flow through timely admissions and discharges in collaboration with medical staff and case management to assure proper utilization of resources. 8. Works collaboratively to solve problems, improve processes and integrate services within the department and with other departments. 9. Develops and implements service initiatives on unit using/tailoring organizational tools and programs such as Because We Care, Our Commitments, etc. 10. Creates and sustains an environment that encourages feedback, problem-solving, self-evaluation and innovation as the way work is accomplished and improvements achieved. 11. Leads patient satisfaction initiatives at the unit level to address metrics identified below target goals. 12. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Hospice Nurse Case Manager

Swan Hospice is actively seeking Registered Nurses (RN) to join our team in New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Job Description A. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems of the patient at home Works closely with other members of the IDT to ensure all patient and family care needs are being met. Provides supervision to Hospice Aides/Homemakers. Education a. Graduate of an accredited registered nursing program. b. Bachelor’s degree in Nursing preferred. c. Certified Hospice and Palliative Nurse (CHPN) certification preferred. 2. Experience a. Has minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience. b. Experience with pain and symptom management interventions preferred. c. Ability to work within an interdisciplinary setting. d. Understanding of Hospice philosophy, principles of death/dying. 3. Licensure, Registration, and/or Certification a. Registered Nurse licensed by the New Jersey State Board of Nursing in accordance with N.J.S.A. 45:11-23 et seq. and N.J.A.C 13:37. b. Valid driver’s license and reliable transportation that is insured in accordance with New Jersey state requirements. c. Satisfactory references from employers and/or professional peers. d. Satisfactory criminal background check.
