FLSA Status Non-Exempt Job Role Summary The Team Captain RN and Triage Nurse RN are positions that allow for a collaborative approach to the patient's healthcare in a multitude of ways. Team Captain RN - This role is for the nurse responsible for the overall care of patients assigned to their team's providers. This includes risk assessment and coordination of care for high risk patients, tracking quality metrics, delivering elements of patient education, and serving as the principal point of contact for patients and other care providers outside of face-to-face appointments. The Team Captain supervises and directs the activities of team nurses and support staff. Triage Nurse RN - Triage is an interactive process between the nurse and patient that occurs over the telephone or face-to-face and involves identifying the nature and urgency of client healthcare needs and determining the appropriate disposition. The role of the triage nurse is to assess the patient, provide patient education, and crisis intervention. The goal of telephone triage services is to direct the caller to the appropriate level of care or service in a safe and timely manner in combination with providing self-care advice and direction. RN-System Operations Center

Overview Evaluate, coordinate, process, screen, document patient transfers into the IU Health System, and collaborates with relevant business partners for appropriate level of care and transport of the patient into or out of the IU Health System. Obtain appropriate medical and demographic information, and coordinate communication between the referring and receiving physicians as well as between healthcare facilities. Perform essential duties related to patient transfer/transport and will coordinate all components of patient care needs during the transfer/transport process. Efficiently engage appropriate departments in the transfer/transport processes, and will assign beds for all scheduled and unscheduled admissions including patient transfers/transports. Associate's Degree is required. Bachelor's Degree is preferred. 3-5 Years of Relevant Clinical experience in an acute care environment. Requires Basic Life Support (BLS), Advanced Life Support (ACLS), and Pediatric Advanced Life Support (PALS) certification through the AHA. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Requires a minimum of 3 years critical care, emergency department, or flight nursing experience or equivalent. Case management/resource management experience is a plus.
