Pay Range: $19.32 - $24.13 Scheduled Weekly Hours: 40 Position Overview This position earns an additional $1.75/hour float differential. ESSENTIAL FUNCTIONS Prepares patients for examination by performing preliminary physical tests; taking blood pressure, weight, and temperature; reporting patient history summary. Cleans exam rooms, obtains lab specimens. Assists the physicians and other staff as needed. Selects appropriate equipment prior to collection to streamline process. Verifies specimen orders per department policy Prepares exam rooms for patient encounters and assists LIP as requested. Maintains appropriate levels of supplies and all relevant forms for any testing performed and maintains statistics as assigned Performs thoroughly all clerical duties as needed, including but not limited to patient registration, answering phones, scheduling appointments, initiating, maintaining and filing medical records and timely distribution of reports. Performs electrocardiograms and other relevant tests including point of care testing following established policies and procedures and universal precautions. Verifies patient information by interviewing patient; recording medical history; confirming purpose of visit. Documents medical information in electronic medical records including medication and refill information and patient call documentation, keeps rooms stocked and organized. Organizes paperwork and filing. Coordinates with front office personnel and ancillary staff in professional and positive manner in adherence of policies and procedures in support of patient service. Assists in front office functions as assigned including but not limited to: Checkout/in duties with claim creation and assuring complete process of discharge paperwork, referrals, patient education, appointments, insurance eligibility etc. Performs other selected clinical and clerical duties that assist in the delivery of primary health care and patient care management under the direction of a provider and manager in accordance with the objectives and policies of Capital Health (CH) and respective state laws governing such action and activities. Cross-trained to perform Patient Reception Rep job duties as needed. New Jersey Prescription Monitoring Program (NJPMP)- uploads a notarized "request for access" form and a copy of their State-accepted Certified Medical Assistant certificate; performs NJPMP monitoring under the direction of the LIP (Licensed Independent Practitioner). Performs injections, subcutaneous or intramuscular; performs duties in accordance with Capital Health policies. Performs other selected clinical and clerical duties that assist in the delivery of primary health care and patient care management within any CH office location under the direction of a provider and manager in accordance with the objectives and policies of Capital Health (CH) and respective state laws governing such action and activities. Performs other duties as assigned. Incumbents working in Primary Care or Specialty Care offices may be assigned to other Capital Health Medical Group Practices as needed. MINIMUM REQUIREMENTS High school diploma or equivalent. Graduate of a post-secondary medical assisting education program accredited by the National Healthcare Association (NHA), or its successor, The Committee on Allied Health Education and Accreditation of the American Medical Association (CAHEA), or its successor; Accrediting Bureau of Health Education Schools (ABHES), or its successor; or any accrediting agency recognized by the U.S. Department of Education. The educational program shall include, at a minimum, 330 clock hours of instruction and shall encompass training in the administration of intramuscular and subcutaneous injections and instruction and demonstration in: pertinent anatomy and physiology appropriate to injection procedures; choice of equipment; proper technique, including sterile technique; hazards and complications; and emergency procedures. Must maintain current certification from the Certifying Board of the American Association of Medical Assistants (AAMA), the National Center for Competency Testing (NCCT), the American Medical Certification Association (AMCA), the National Association for Health Professionals (NAHP), the National Certification Medical Association (NCMA), or registration from the American Medical Technologists (AMT), or any other recognized certifying body approved by the board. First Pre-Admission Testing, located in 3 Cooper calls to review a patient’s health information prior to a surgical procedure. Once approval for the recommended surgical procedure is finalized and patient information is reviewed the patient is provided with information on when and where to report. Once arrived and registered the patient is escorted to the appropriate location to be prepped for surgical procedure. These RN’s prep the patient for the procedure. They review medical history, medical procedures and place an IV. Patients are met by Anesthesia and their Surgeon. Once everything is reviewed and verified the patient then goes to the OR. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. JOB SUMMARY The certified nursing assistant is an integral member of the health care team who delivers quality care through collaboration and communication with team members. They work under the direct supervision of the registered nurse to support and assist the nurse in the delivery of quality care to patients and families. Some of the major responsibilities will include collection and reporting of patient data, incorporating organizational competencies of superior quality care and assuming responsibility and accountability for their role performance outcomes while supporting GSPP's Standards of Excellence. ESSENTIAL FUNCTIONS PATIENT / CUSTOMER Essential Accountabilities Report Receives written/verbal report at the beginning of each shift from the registered nurse (RN) regarding the care that is to be delivered to the assigned group of patients, or for patient needing constant observation. Gives report when leaving unit, and at the completion of the shift regarding the care that was delivered; any abnormalities that occur during the shift are to be reported as they occur to the RN as well as to the oncoming CNA. Assists in Assessment Identifies the patient per hospital policy before carrying out functions. Assists with data collection for nursing history and physical e.g. vital signs, ht. wt, blood sugar results, functional status and communicates changes to the RN in a timely fashion. Participates in respectful communication with patients, families and coworkers. Provides patient and family information to the registered nurse for coordination of care and discharge planning. Observes skin changes and reports discoloration, abrasions, infestations or skin breakdown. Planning Assists in setting mutually agreed upon realistic individual patient goals. Integrates the RN’s directions re: the patient’s plan of care into the care given to the patient. Gives RN report before the end of the shift regarding the patient’s response to the plan of care issues, and at various times during the shift to report unusual findings e.g. abnormal blood sugar results, vital sign changes, pain etc. Intervention Promotes infection control by using the appropriate hand washing techniques. Follows standard precautions and any other isolation precautions as dictated by the patient’s condition.
