How you make a difference As a Nurse Practitioner MAT, you will deliver comprehensive medical care to patients, operating within established policies and procedures and in alignment with state regulations, under the guidance of the Site Medical Director and fellow site Physicians. Your role involves meticulous documentation of provided services for monitoring purposes and promptly communicating any schedule changes or coverage needs to the Medical Director and site director to ensure uninterrupted medical services for patients even during your absence. Key Responsibilities Conducts thorough medical examinations of referred patients upon reporting to the assigned facility, adhering to clinic treatment policies and all relevant state and federal regulations. Provides guidance and training to medical staff, participates in case conferences, and collaborates with clinic personnel on treatment plans while ensuring accurate and timely documentation of patient progress in SOAP format. Responds promptly to health emergencies following standard protocols, reviews records as necessary, and utilizes available resources for problem resolution before considering off-site referrals. Performs comprehensive medical assessments, including history-taking and physical examinations, to diagnose opioid dependence and coexisting medical or psychiatric disorders, making appropriate referrals for further treatment. Prescribes tailored patient care regimens, upholding confidentiality standards and contributing insights to facility committees upon request from the Site Medical Director or Health Services Administrator. How you make a difference The Nurse Discharge Planner LVN develops and coordinates clinically informed reentry care plans for CalAIM-eligible individuals to address medical, behavioral health, and substance use needs prior to release. The role conducts basic nursing assessments, reviews health records, coordinates referrals, and facilitates warm handoffs to managed care plans and community providers. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. Compensation Salary Range: $23.29 - $31.94 / hour The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. How you make a difference The Licensed Vocational Nurse (LVN) provides assistance and collects data in the delivery of patient care in a correctional setting, utilizing the nursing process of assessment, planning, implementation, and evaluation. As part of this role, the LVN directs and guides patient teaching and activities in accordance with their education, demonstrated competencies, and the Nursing Practice Act (NPA). The LVN works closely with other healthcare professionals to ensure high-quality care and compliance with established policies and procedures. Key Responsibilities Assist in patient assessment and treatment planning in accordance with the Nursing Practice Act and Correctional Health Services policies. Administer medications and implement medical plans as directed by the Health Care Practitioner, utilizing proper techniques and procedures. Respond to emergencies and maintain safety through the implementation of established protocols and precautions. Implement nutrition and therapeutic diets and assist in medical procedures as necessary to meet patient needs. Document nursing encounters and communicate information to healthcare staff and supervisory personnel. Additional Details Great Team & Benefits!
