Responsibilities and Initiatives To help NHF meet its growth goals, the Medical Assistant will: Conduct intake and discharges for our guests. Complete registration packets with our guests for clinic visits. Manage a caseload of guests under the direction and supervision of the Medical Coordinator/LVN. Conduct rounds in our facility, obtaining vital signs, assisting with blood sugars checks and or information as needed. Triage guests and prepare them for nurses’ assessment, updating guest charts daily using CRM database system, and scheduling appointments with them. Monitor medication management based on Nursing plan of care and reporting back to LVN if guests are in need of teaching, or medication oversight. Assist with providing on-site covid antigen testing. Assist Nursing manager as needed. Coordinating and providing linkages for guest’s medical appointments and follow-up. Participating in weekly internal and external case conferencing, board review team meetings, monthly staff meetings, monthly all site nursing meetings, trainings, and in-services. Performing administrative tasks such as charting, guest summaries and answering phones. Coordinate transportation for medical related matters. Good knowledge of scope of practice for Medical Assistants. Completes additional tasks/duties as required to meet the needs of the facility, not to impede primary responsibilities. Adhere to the schedule given, modifications, and/or absences must be communicated as outlined with your leader· Always follow NHF policies and procedures. Medical Assistant certification required. Graduate of an accredited Medical Assistant program. Bilingual in Spanish (preferred). Ability to work independently with strong time management and organizational skills. Attention to detail and high accuracy are a must. Excellent verbal and written communication skills as well as a high level of professionalism required. Experience in medical office (preferred). Job Description The University of California, San Francisco (UCSF) Advanced Heart Failure Comprehensive Care Center’s Mechanical Circulatory Support (MCS) Program seeks a full-time ECMO Specialist Nurse to help grow our program. The AHFCCC team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, researchers and others. Our MCS program is nationally known for cutting edge MCS support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The ECMO Nurse Specialist is a highly skilled critical care nurse responsible for the initiation, management, and monitoring of Extracorporeal Membrane Oxygenation (ECMO) support for critically ill patients. Working as part of a multidisciplinary team, the ECMO Specialist ensures safe, effective circuit operation, timely response to complications, and adherence to established clinical protocols in order to optimize patient outcomes. The Extra Corporeal Membrane Oxygenation (ECMO) Nurse Specialist will be responsible for providing device related care to patients on ECMO as well as other temporary and durable MCS devices in the inpatient, operative and inter hospital setting. Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. Treatment Nurse Registered Nurse (RN) Licensed Vocational Nurse (LVN) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
