Employment Type: Part time Shift: Evening Shift Description: Mother Baby Unit East Tower, 2nd floor This unit has 40 private rooms for both high risk and low risk postpartum patients, post-operative surgery and medical patients. As part of MercyOne Children’s Hospital the unit also has a newborn nursery. Nursing and medical providers work together as a team to deliver care for the newborn, mother as well as medical patients. POSITION PURPOSE Acts as a clinical/educational resource to patient care activities/teaching programs within the area of defined specialization. Supports patient/family/unit needs of diverse ages. ESSENTIAL FUNCTIONS Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions. Identifies educational needs, develops program(s) to meet those needs, and coordinates implementation of the program(s). Evaluates programs and modifies as indicated. Works independently with limited direct supervision using the Nurse Practice Act, MercyOne Medical Center policies, nursing standards, and professional judgment. Serves as a resource in area of specialty and collaborates with other disciplines. Provides consultation in specialty areas to all levels of health care providers throughout the hospital, affiliated schools of nursing, and the community. Actively participates on committees, task forces, and in work groups, as requested . Participates in research by reviewing literature, communicates findings, identifies potential research questions/projects and coordinates research projects in area of clinical specialty. Performs efficiently in emergency situations, following established standards. Presents selected program topics. Facilitates program presentation/equipment gathering. Utilizes delegation skills effectively. Coordinates the lactation champions and the “Mommy and Me” class. Other duties as needed and assigned by the manager. Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. A Brief Overview Prescribes, delegates and coordinates professional nursing care to pediatric and adult congenital heart patients through the use of nursing process and in accordance with hospital policy/procedure. Essential Functions Implements a comprehensive plan of care for the cardiology patient/family: Teaches the patient/family about all aspects of congenital heart disease such as anatomy/physiology, medications, blood work, activities, and follow -up. Communicates the medical plan and related nursing care implementations to PCP and other medical resources involved in patient's care. Monitors outpatient lab work and reports findings to cardiologist as needed. Authorizes prescription refills under provider supervision. Provides outpatient cardiology clinic nursing coverage Reviews patient chart to assure outpatient testing is correct and appropriate. Staffs outreach clinics with cardiologists as assigned, traveling required. Acts as liaison between pediatric cardiology, patients and their families and other health care team members: Communicates the cardiologist plan of care to the patient/family. Schedules diagnostic tests and procedures when necessary, including prior authorizations. Coordinates necessary lab work, follow-up testing, and communicates any medical changes to the patient/family. Refers patient needs/concerns to the cardiologist for medical follow-up. Acts as patient advocate to address the patient’s needs and concerns. Serves as the resource for answering patient/family questions or concerns regarding medical plan of care. Works with referring physicians in coordinating and implementing patient care issues and scheduling of cardiology procedures. Attends weekly cardiac catheterization conferences in person or via zoom, attends cardiac educational offerings when appropriate Regular attendance at work is an essential function of the job. Required Qualifications: For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations Active and unrestricted driver's license In addition to a driver's license, access to reliable transportation Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) Ability to gain a collaborative practice agreement, if applicable in your state Ability to navigate varied environments; ability to sit/stand/kneel as needed to perform duties Preferred Qualifications: 1+ years of clinical experience as an APC (family, geriatric, or home health preferred) Experience in geriatric medicine, long-term care, senior living or home care setting Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficiency with electronic medical records and technology Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
