Key Responsibilities Direct Patient Care Accept patient care assignments from the Charge RN or designee Immediately report changes in patient condition to the primary RN Obtain, document, and report vital signs, rechecking as needed Measure and document patient intake and output Perform capillary blood glucose testing and report abnormal results Perform ECGs as directed Apply monitoring equipment including NIBP, ECG electrodes, pulse oximetry, and end-tidal COâ‚‚ monitoring Patient Support & Unit Assistance Assist with patient transport and transfers throughout the hospital Respond to patient call lights and family requests in a timely manner Support activities of daily living (ADLs) under nursing supervision Help maintain a safe, clean, and supportive patient care environment Perform unit-related tasks assigned by the Nurse Manager or designee Patient Safety & Observation Provide continuous observation of assigned patients to ensure safety Verbally redirect patients engaging in at-risk behaviors Maintain patient privacy, dignity, and rights Virtual Safety Attendant / Telesitter Program When assigned, the Patient Care Technician may function as a Virtual Safety Attendant supporting the hospital's Telesitter patient monitoring program . Cantu Concussion Clinic Medical Director and the Post Acute Service Line Director and in direct collaboration with supervising physicians, performs duties to assist supervising physicians in a wide range of clinical tasks. Responsible to manage a wide range of outpatient clinical duties, as well as emergency situations, while adhering to standard practices and established procedures. This position ensures the quality and safe delivery of care to patients of all ages. Job Functions Performs complete admission history and physical exams, and daily notes on assigned clinic and center patients. Evaluates, manages and treats concussion patient as well as assists with center patients. Facilitates the communication of evaluations with primary care physicians. Develops and initiates the plan of care during the office examination and collaborates with physicians and other healthcare professionals and family members. Performs a complete concussion assessment including history of all previous concussions, risk factors, and symptoms as well as current concussion assessment. Differentiates normal test results from abnormal findings such as neuropsychological test results, MRIs and CTs. Identifies wellness and risk factors; communicating relevant data and providing complete, accurate written documentation in the patient's medical record. Obtains appropriate authorizations required for all treatments and medication orders. Writes and signs preliminary intake assessments and outpatient forms as allowed, including necessary medical care instructions. Educates and actively involves patient and family members in care. Identifies and manages psychological patient issues and refers appropriately. Takes patient and family member phone calls and advises as necessary in between patient appointments. Provides appropriate and timely communication with the referring physicians and other referring healthcare personnel. Order tests and procedures as indicated. Ensures that treatment plan is individualized based on the family and patient needs. Prescribes, in collaboration with or at the direction of the Medical Director and Assistant Medical Director, medications specific to the patient's diagnosis and level of medication tolerance. Assists Medical Director in educating clinic staff Works with area schools both private and public in education on concussion. Assesses and evaluates concussion patients at area private schools as directed by Medical Director. Maintains and supports patient's rights to care, privacy and respect. Other duties as assigned from time to time. Minimum Qualifications Education Completion of a Masters' Degree in Nursing and an accredited Nurse Practitioner or Physician Assistant Program required. Experience Previous experience with concussion patients strongly preferred. Previous experience with Sports Medicine patients strongly preferred. Previous experience in the area of neurology strongly preferred. Licensure and/or Certification Current license as a Nurse Practitioner or Physician Assistant in Massachusetts required. This practice specializes in the care of patients with multiple myeloma as well as patients requiring specific procedures in our procedure clinic. These teams have a demonstrated history of providing exquisite, evidence-based patient care. This is a fantastic opportunity for an Advanced Practice Provider (NP or PA) to work in this invigorating and collaborative environment. This APP will deliver direct care to outpatients with two days of the work week dedicated to working directly with the multiple myeloma team and two days dedicated to working in the procedure clinic. With an interdisciplinary team approach, this provider will promote a collaborative relationship between providers, professional nurses, and administrative staff, to ensure continuity of patient care in the management of their disease and related issues as well as to develop expertise in the field. Within this role, the advanced practice provider will be responsible for a panel of patients. Other responsibilities include: • Caring for patients throughout their continuum of care and fostering collaboration with other providers within the hospital and clinics to facilitate patient care. • The APP develops, implements, and evaluates a plan of specialized health care for this oncology patient population throughout the care continuum. • Promotes a professional and collaborative relationship and effort between the advanced practice team, medical staff, professional nursing staff, and administrative staff. • Strives to provide and to ensure continuity of care for patients, while being efficient in use of resources. • Is willing to obtain procedural proficiency for oncologic specific procedures including bone marrow biopsies, lumbar punctures, Ommaya reservoir taps, and skin biopsies, amongst others. Training will be provided. • Indications and interpretation of radiology, pathology, and laboratory results. • Understands the complex medical environments of cancer patients including, but not limited to, those with advanced malignancy or organ dysfunction. • Serves as a role model for professional advance practice and participates in the professional development of the team through teaching, committee involvement or quality and safety initiatives to name a few. • In addition to clinical skills, this person should possess strong leadership and communication skills and be able to work effectively with other members of the team. Ideal Candidate: • 2 years Advanced Practice experience preferred; prior hematology/oncology experience preferred. • Proficiency with Epic and EMRs preferred. • Procedural competency preferred. • Familiarity with clinical trials preferred.
