SUMMARY: The Wound Care Nurse can be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN) and is responsible for providing primary skin care to residents under the medical direction and supervisor of the resident’s attending physician, the Director of Nursing, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. ESSENTIAL DUTIES AND RESPONSIBILITIES: Confer with the Director of Nursing and/or other licensed nursing personnel regarding the dermatologic disorders of residents in the facility Examine the resident and his/her records and charts, and discriminate between normal and abnormal findings, in order to recognize when to refer the resident to a physician for evaluation, supervision, or directions Provide assessment and diagnostic services to residents. Perform an assessment evaluation using techniques including observation, inspection, and palpation Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff Identify, manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic, and viral skin infections, scaling popular diseases, and benign tumors Provide preventative health care services. Make recommendations to the resident concerning nutrition, prevention of common skin disorders, etc., Make written and oral reports/recommendations to the attending physician, Medical Director, or the DON concerning the status and care of the residents Implement and maintain established policies and procedures relative to skin care treatments. Interpret these to the physician, resident family members, and public as appropriate Performs other duties as assigned QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. ESSENTIAL DUTIES AND RESPONSIBILITIES: Understand a client’s care plan and adequately implement it. Assist clients according to their care plan which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, transferring, toileting, and protective supervision. Notify the Care Manager of client concerns, changes in care or client condition and client emergencies. Notify the Care Manager and/or Staffing Coordinator of any safety concerns. Follow company Infection Control practices; notify the office if in need of appropriate Personal Protective Equipment (PPE) or supplies. Use the WellSky App Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the client’s home. Conform to the company dress code and the no drug/alcohol use policy as specified in the Employee Handbook. Report to work on-time and ready for the day. Complete ongoing training as required by client care needs and/or changes in company policies and procedures. Be responsive to the Care Manager.
