About the Opportunity This unique role combines outpatient valve clinic management with inpatient structural heart care coordination. The APP will participate in all aspects of the patient journey, including initial consultation, diagnostic workup, multidisciplinary Heart Team evaluation, procedural planning, hospitalization, discharge coordination, and follow-up.
