Description The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Assures a completed discharge instructions, medication reconciliation and provide information to patient/family regarding the use of the patient portal and other pertinent resources. Patient Education/Ethic and Diversity Supports patient/family/spiritual/cultural needs when assessing the patient and family needs for teaching; identifies the patient's learning level and readiness, and documents education record. Utilizes the teach back methodology for advancing patient education about medication side effects and new medication education. Utilizes and distributes Krames for patient education /clinical condition. Utilizes culture vision as a resource in care planning and delivery. Treats patients, families and staff in a caring manner with respect and dignity. Acknowledges and values the contributions of patient’s significant others as an integral part of the plan of care. Adheres to ethical standards of practice. Calls for an ethics consult when appropriate. EBP and Research Implements nursing interventions consistent with the established plan of care based on evidenced based care. Maintains and advances the professional image and science of nursing and the practice of professional nursing through use of best available evidence utilizing resources such as Ovid/Joanna Briggs, Up to Date, Culture Vision, and other reliable sources of evidence. Resource Utilization Ensures an appropriate level of care order is documented in the EMR for all admissions. Understands and adheres to the charging of supplies. Use of Technology Assures all electronic and paper documentation is complete. Utilizes the guardrails for the Alaris pump. Understands physiologic monitoring and the implications to drive changes to clinical management of the patient. Utilizes the bed management system Identifies new technologies for unit, hospital or system use. Participates in CEOC activities for the advancement and optimization of the medical record as requested. JOB SUMMARY Responsible to plan, organize, develop, direct and delegate (as appropriate) the overall operation of the Nursing Department in accordance with current federal, state, and local standards, guidelines and regulations, and facility policies. QUALIFICATIONS Must possess a current, unencumbered license to practice as a Registered Nurse in the State of Illinois and/or Wisconsin. Minimum of one (1) year nursing service administration experience. Must possess current CPR certificate or become certified within two weeks of employment. Develops and implements a nursing/rehab/restorative philosophy that promotes quality nursing, rehab, restorative care for each resident. Assure adequate staff on all shifts. Audits charts and EHR to ensure daily documentation is done. Assure laboratory and x-ray reports are received and filed for all tests completed. Audits transfers/discharge information to assure accuracy and completeness. Audits charts for deficiencies. IDT-through regularly scheduled chart audits. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in resident care plan conference when indicated. Attend Customer and or Family Council meetings when invited. Report all concerns, complaints made by the resident or their family to the Administrator. Recruit, interview, and select the Ladies and Gentlemen of Alden with the approval of the Administrator. Schedule department work hours, personnel, work assignments, etc., to expedite work. Ensure a sufficient number of Ladies and Gentlemen of Alden are available to meet the needs of our customers. Counsel/discipline as requested or as necessary. Complete performance evaluations timely. Make daily rounds to ensure nursing personnel are performing required duties and to ensure that appropriate procedures are being followed. Make physical rounds on all customers daily. Make rounds with physicians as necessary. Submit Workmen’s Compensation forms (once reviewed) to the Business Office Manager within 24 hours after their occurrence. Plan, conduct and participate in In-service Education Programs. Assist in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department and its policies and procedures. Attend and participate in continuing education programs. Place orders for equipment and supplies as necessary following established budgetary guidelines. Maintain budgetary restraints by following established procedures for preparing purchase orders for review by the Administrator prior to placing orders. Maintain a reference library of written nursing materials (i.e., Drug Formulary, State and Federal regulations, etc.) that will assist the nursing service department in meeting the day-to-day needs of the customer. Participate with the Administrator and Admissions Director in selecting appropriate admissions and assignments of rooms. Review nurses’ notes/EHR to ensure they are informative and descriptive of the nursing care being provided, and they reflect the customer’s response to the care. Monitor medication passes and treatment schedules to ensure medications are being administered as ordered and treatments are provided as scheduled. Ensure that professional nursing personnel have valid and current licenses as required by State. Ensure changes in customer condition are reported to family and attending physicians. Attend Department Head meetings, Infection Control meetings, Quality Assurance meetings, etc., as directed. Perform other related duties as assigned. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner.
