This position ensures the appropriate utilization of clinical resources with a goal of a safe and timely discharge for the patient. This role navigates health system services to support effective transitions while advising the team on healthcare industry compliance. The RN Case Manager must be adept at driving throughput metrics, clinical effectiveness, and fiscal responsibility. Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Tasks: Reviews all cases within 24 – 48 hours or the next business day of admission/bed placement and each day throughout the stay to facilitate care progression to establish an anticipated length of stay and transition planning needs. Add key accountabilities here. Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Assess home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations. Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assess patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Make the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revise the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Must be able and willing to travel 50% Previous Home Health or Hospice Experience is preferred Previous experience with HomeCare HomeBase is preferred Whether it’s 4 hours or 24, we’re flexible—we just need consistency.
