Job Description Summary: An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home, or in an alternate infusion site, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under very close direct supervision of the designated supervisor. Job Description: Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals. Participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Naven Health’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Naven Health’s policy. Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Naven Health’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Naven Health’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Naven Health information including adherence to HIPAA regulations. Speaks knowledgeably about Naven Health’s scope of services and effectively instructs patients about related plan of care. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing on-call responsibilities as applicable. Performs other related duties as directed by supervisor. Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel with proof of liability insurance. The Graduate Resident Nurse utilizes the nursing process as the frame of reference for practice and provides direct patient care, with supervision from a registered professional nurse to patients and families. This is an entry-level nursing position for Graduate Resident Nurses. Training, evaluation, and competencies for this role are overseen by a Nurse Leader in the department. During the first year of employment, you will be enrolled in a Nurse Residency Program. Nurse Leadership will determine a balance of Clinical and Educational time. Job Description: 6 Southeast is a 20 bed Medical Surgical Telemetry Unit providing expert care for a variety of diagnoses with a focus on cardiac and respiratory conditions. We are the designated unit for patients recovering from Feminizing Gender Affirming surgeries and have become passionate advocates for the transgender population. This diverse environment creates an ideal setting for learning opportunities and growth. Essential Responsibilities including but not limited to: I. Clinical Judgment and Decision Making: Ways in which nurses come to understand the problems, issues or concerns of patients/families, to attend to relevant/critical information, and to respond in concerned and involved ways. A. Graduate Resident Nurse is expected to and is accountable for providing safe patient care by demonstrating organizational skills that maintain and coordinate safe delivery of quality care for assigned patients/families. Develops a culturally competent plan of care that identifies patient problems, expected outcomes, and addresses preventative measures. B. Performs systematic patient and family assessment relevant to practice settings. C. Delivers care that is specific to the age of the patient. D. Evaluates effectiveness of care and adapts plan based on patient/family response. II. Professional Relationship: The professional relationship is based upon Lahey Clinic’s guiding principles and positive effective communication. This relationship extends beyond formal assessment to integrate the particular patient’s response and his/her religious/spiritual, ethnic and cultural beliefs into the plan of care. A. Establishes professional relationship with nurses, physicians and other colleagues and patients and families. The relationship includes the patient and family as a partner in care and is based upon Swanson’s Theory of Caring. B. Educates the patient and family about the patient’s illness and treatment including preventative measures appropriate to the patient. C. Supports Lahey Clinics commitment to community-based activities both within the Lahey community and beyond. III. Clinical Leadership: The Graduate Resident Nurse supports the development of others and creates a practice climate of responsiveness and learning based on Benner’s Novice to Expert skills acquisition model. This supports the retention and recruitment of clinically competent nurses. The ability to lead and influence the reasoning of colleagues in the patient’s best interest depends on skillful comportment, authoritative clinical knowledge and wisdom, the conviction to act as an ethical agent despite adverse consequences, and the ability to listen and enhance others’ strengths. A. Demonstrates professional behavior by adhering to unit policies and procedures, practice guidelines specific to the setting and the ANA Code of Ethics. B. Demonstrates skills as a responsive team member. C. Demonstrates role of nurse as teacher. D. Demonstrate ability to fulfill role in Regulatory compliance and readiness. IV. Clinical Scholarship: The Graduate Resident Nurse achieves clinical scholarship through experiences with patients, through collaboration with colleagues and leadership and through formal education. Learning is transformed into knowledge through self-reflection and analysis. A. Upholds standards of quality nursing care. B. Demonstrates a commitment to lifelong learning. C. Demonstrates evidenced based practice (EBP) in nursing. Essential Duties including but not limited to: Demonstrates the ability for decision-making by integrating theoretical knowledge with practical experience in caring for patients. Administers medications and treatments. Coordinates and assists in the care of patients and directs assistive personnel in order to provide safe, effective, efficient, equitable, and timely patient-centered care. Demonstrates initial awareness of environmental forces on health care of patients and their families. Develops a collegial and collaborative relationship with other professionals to determine healthcare needs of patients and families. Develops relationships with patients and families to maintain and communicate trust and respect. Communicates effectively in the exchange of information. Demonstrate the ability to act as a patient advocate. Maintains all mandatory education requirements, include emergency skills and unit-based competencies. Identifies needs for continued growth and development in conjunction with the clinical practice specialist. Demonstrates a commitment to patients, staff, and to LHMC. The individual’s nursing practice reflects the goals of the hospital. Demonstrates responsibility and accountability for own nursing practice and patient safety. Perform all other duties as needed or directed to meet the needs of the department. Minimum Qualifications: Education Graduate of a state approved and/or accredited School of Nursing. Bachelor of Science in Nursing (BSN) preferred. Licensure, Certification, Registration Current license to practice professional nursing from the Massachusetts Board of Registration; graduating nursing students must provide NCLEX registration documentation when eligible. American Heart Association - Basic Life Support Certificate Experience 0-1 years related work experience required. Basic familiarity with computers. Ability to navigate at a basic level within web-based applications. Skills & Abilities Demonstrates the necessary clinical skills and technical knowledge to provide care for patients according to Nursing Policy and Procedures. Demonstrates the necessary clinical skills and knowledge to provide care for patients according to the unit-specific competencies. Excellent interpersonal and organizational skills. Pay Range: $37.00 - $37.00 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
