Key Responsibilities: Conduct comprehensive, face-to-face clinical assessments for current participants in accordance with UAS-NY guidelines, agency protocols, and New York State regulations. Assess prospective participants by reviewing clinical, psychosocial, and environmental factors to determine appropriate levels of care. Collect and interpret clinical data from various sources to support sound, evidence-based care decisions. Develop and update individualized service plans tailored to participant needs. Demonstrate understanding of the complex needs of individuals with chronic medical and bio-psychosocial conditions.
