Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care. Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition. Participate in patient care and make nursing visits as needed to support the clinical team. Assist with reviewing clinical documentation for completeness, accuracy, and compliance. Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation. Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families. Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team. Serve as a clinical resource and role model for nursing staff. Participate in interdisciplinary group meetings and other clinical meetings as assigned. Respond appropriately to urgent clinical situations and assist with escalation of patient-care concerns. Qualifications Current, unrestricted Registered Nurse (RN) license in the State of New Jersey. Associate Degree in Nursing required; BSN preferred . Minimum of 2–3 years of professional nursing experience . Previous hospice, home health, palliative care, or end-of-life care experience strongly preferred. Previous leadership, charge nurse, preceptor, or supervisory experience preferred. Current CPR/BLS certification. Valid driver's license, reliable transportation, and appropriate automobile insurance. Strong clinical assessment and critical-thinking skills. Excellent written and verbal communication skills. Strong organizational and time-management abilities. Ability to work independently while functioning effectively as part of an interdisciplinary team. Proficiency with electronic medical records and standard computer applications. Registered Nurse needed in Brooklyn, full-time position! We are seeking a Registered Nurse (RN) for their Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The Registered Nurse (RN) provides comprehensive nursing services across the CRTP, ICM, and CTI. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Salary : $95,000 per year plus benefits. Schedule: Monday - Friday 9am-5pm Essential Responsibilities (including but are not limited to): Clinical Care -Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. -Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. -Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. -Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. -Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. -Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. -Monitor and manage medication storage and disposal procedures in accordance with federal and state law. -Develop and monitor health goals that are person specific. Review, update, and revise these documents to ensure they are reflective of the current person supported status. -In compliance with OSHA mandates, monitors infection control, environmental changes, and ensures that all guidelines regarding universal precautions are appropriately followed. -Order and maintain necessary first aid and medical supplies as needed. -24/7 availability via phone for consultation and basic direct care. Case Management & Transitional Support -Partner with ICM teams to address both medical and behavioral health needs in service planning. -Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness. -Facilitate hospital discharge and community reintegration through CTI, ensuring continuity of medical, psychiatric, and social support. -Coordinate follow-up care with primary care providers, specialists, and community-based resources. Collaboration & Training -Serve as the clinical liaison among providers, families, and program staff. -Participate in interdisciplinary treatment planning and case conferences. -Train and mentor direct support professionals (DSPs) on health-related tasks and emergency response. -Maintain timely and accurate documentation in compliance with OPWDD, OMH, and DOH regulations. Overview Provides licensed practical nursing support to the Medical Care at Home (MCAH) clinical team during in-house and virtually supported preventive assessments. The LPN, Field Clinical Support is physically present with the patient and works under the direction of the Nurse Practitioner (NP), collecting and communicating objective clinical information, completing structured intake activities, supporting Care GAP closure, performing ordered procedures and testing within LPN scope and demonstrated competency, and reinforcing patient education. The NP maintains responsibility for the clinical assessment, interpretation of findings, diagnoses, Assessment and Plan, and final encounter documentation. • Prepares patients for MCAH preventive and comprehensive assessment visits and completes structured intake activities, including collection of medical history, medication information, vital signs, functional information, and other objective clinical data for NP review. • Works under the direction of the NP during virtual encounters and communicates objective findings, changes in patient condition, and other clinical observations requiring NP review. • Supports medication reconciliation by comparing available medication information with patient-reported use, identifying discrepancies, and communicating findings to the NP for assessment and follow-up. • Performs ordered clinical procedures, screenings, specimen collection, point-of-care testing, and other nursing activities that are within LPN scope, organizational policy, and demonstrated competency. • Performs phlebotomy and ensures specimens are collected, labeled, handled, stored, and transported according to established protocols when applicable. • Supports completion of identified Care GAPs by reviewing assigned preventive needs, facilitating ordered testing, documenting completion, and communicating barriers or outstanding needs to the NP and appropriate team members. • Provides NP-directed examination support and reports objective clinical findings in real time during the virtual encounter. The LPN does not independently diagnose, triage, or establish the clinical Assessment and Plan. • Utilizes TytoCare or other approved remote examination technology when available and clinically indicated to facilitate additional NP examination. When further examination cannot be appropriately completed during the visit, supports coordination of follow-up as directed by the NP. • Recognizes and promptly reports abnormal findings or changes in patient condition to the NP. Follows established escalation and emergency protocols, including activation of emergency services when directed or clinically required by protocol. • Provides and reinforces patient and family health education, preventive health information, specimen collection instructions, medication-related teaching, and NP recommendations within LPN scope and organizational policy. • Accurately documents patient information, nursing activities, clinical observations, procedures, testing, and communication in the electronic medical record (EMR). • Ensures documentation is complete, timely, accurately identified, and includes required signatures, dates, orders, and supporting information. • Travels to patient homes, community settings, or care facilities and adapts to varied field environments while following safety and infection prevention standards. • Transports, operates, cleans, and maintains required clinical equipment and supplies in accordance with manufacturer guidance and organizational policy. • Participates in interdisciplinary team meetings, case conferences, clinical supervision, education, and competency activities to support continuity and quality of services. • Communicates with Care Managers, Primary Care Providers, scheduling staff, and other members of the care team to support referrals, follow-up, and completion of the NP plan. • Supports daily operational needs of the VNS Health-affiliated Medical Care at Home practice and contributes to quality, productivity, Care GAP closure, and performance goals. • Participates in special projects and performs other duties as assigned. • The LPN collects, documents, and communicates clinical information within LPN scope and competency; the NP performs the clinical assessment and interprets the findings. • The NP is responsible for diagnoses, clinical decision-making, the Assessment and Plan, and determining whether additional examination or escalation is required. • If additional examination is needed beyond what can be appropriately completed during the LPN-assisted virtual visit, the NP determines the next step. This may include a TytoCare-facilitated follow-up, an in-person NP visit, PCP/specialist follow-up, urgent evaluation, or emergency services based on clinical urgency. • The LPN does not independently perform medical diagnosis, independent triage, or independently establish the patient's plan of care. • Participates in special projects and performs other duties as assigned. PRIMARY FUNCTIONS : q Makes decisions relevant to the operation and management of the facility during the assigned shift. q Verifies that the resident's physician and family is notified when there is a significant clinical development in the resident's condition or if the resident requires immediate medical care. q Makes decisions concerning staffing in the facility such as replacing call outs, authorizing overtime and reassigning personnel when appropriate. OTHER DUTIES: q Assumes independent responsibility for yearly written evaluations of charge nurses and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Such evaluations become part of the charge nurses' permanent records and shall be used as the basis for determining continued employment, promotional opportunities and the amount of any wage increases granted. q Assumes independent responsibilities for documenting disciplinary concerns for charge nurses on designated shifts, including reprimands, oral and written warnings, and recommending the suspension or discharge of an employee. q Directs proper handling of resident care emergencies. q Makes house rounds and consults with charge nurse concerning new admissions and acutely/seriously ill residents. q Ensures availability of supplies and that all equipment is in proper working order. q Supervises to ensure the proper completion of accident/incident documentation and notification. q Functions as Charge Nurse RN in the event that there is a call out and the nurse cannot be replaced. q Assists in on-going Quality Assurance of resident care and documentation. q Assumes other duties as assigned by ADNS. q Conducts in-services during assigned shifts according to pre-determined schedules. Records attendance. q Monitors schedule of physicians' visits and keeps Medical Director advised of any and all discrepancies. q Projects the facility's image in a positive light to residents, staff and community. q Maintains a positive attitude and rapport with family members. q Involved in medication returns EXPECTATIONS : q Presents a neat appearance with a pleasant speaking voice and personality.
