Location New York, New York Shift: Day (United States of America) Description: High Quality, Patient Centered Affordable Care The Ambulatory Care Network (ACN) is part of the Division of Community and Population Health of New York-Presbyterian, one of the nation’s most comprehensive academic health care delivery systems affiliated with two world-renowned medical schools. Care Management Coordinator – RN – Ambulatory - Days Pursue this highly-transformational role, responsible for providing care management to high risk patients in the local community to improve the management of their chronic illness and to decrease unnecessary emergency and inpatient admissions. Conduct case-finding, assessments, education, counseling and other interventions to patients who are targeted through chronic disease registries. Assist patients in navigating NYP service delivery system and other services to overcome barriers to self-management and adherence. Connect patients and their families to community resources and ensure the proper continuation of quality care. This is a full time position working five (5) 7.5-hour workdays between the hours of 8:00am-6:00pm at New York-Presbyterian/Columbia University Irving Medical Center.
