Job Summary: We are seeking a Nurse Practitioner (NP) with a focus in Psychiatry to provide care in our 20-bed geriatric and older adult inpatient psychiatric unit at Clinton Hospital. As part of a multidisciplinary team, the NP will be responsible for conducting psychiatric evaluations, diagnosing mental health conditions, prescribing medications, managing treatment plans, and collaborating with psychiatrists, social workers, and nurses to provide comprehensive care. This role also involves participating in the development of care protocols, patient discharge planning, and patient/family education. The Nurse Practitioner will work closely with the Medical Director and other members of the psychiatric team to ensure the highest quality of patient care, safety, and compliance with all regulatory standards. Key Responsibilities: Conduct comprehensive psychiatric evaluations of patients, including history-taking, mental status exams, and diagnostic assessments. Diagnose a wide range of psychiatric conditions, including mood disorders, anxiety, psychosis, addiction-related disorders, and dementia. Develop individualized treatment plans in collaboration with the multidisciplinary team, focusing on pharmacological and non-pharmacological treatments. Prescribe psychiatric medications and monitor patients for effectiveness, side effects, and any necessary adjustments. Participate in daily treatment team meetings, providing input on patient care and treatment progress. Collaborate with psychiatrists, social workers, nurses, and other healthcare professionals to ensure coordinated care. Support patients and families with education on psychiatric conditions, medication regimens, and recovery strategies. Provide ongoing management of patients, ensuring continuity of care through discharge planning and follow-up. Ensure documentation of patient assessments, progress, and treatment plans is complete and accurate in the electronic health record (EHR). Stay current on best practices, treatment modalities, and advancements in psychiatric care. Participate in performance improvement initiatives and quality assurance processes to improve patient care and outcomes. He/She will also act as an expert witness for court cases involving civil commitments and guardianships Qualifications: Education: Master’s Degree in Nursing (MSN) from an accredited Nurse Practitioner program. Licensure: Current Nurse Practitioner license in the state of Massachusetts. Certification: Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) is required. Experience: Prior experience in psychiatric settings is preferred, including inpatient care, medication management, and crisis intervention. Preparing room for admission and ensuring patients have appropriate identification bands upon admission and throughout their hospitalization. • Transporting patients and their belongings and their equipment and supplies, as needed and as part of the transfer/discharge process. • Providing personal care to patients including, bathing and skin care, shaving, mouth care, grooming, perineal care and changing linens, as per hospital protocols. • Providing basic skin care including but not limited to: back rubs, routine diabetic foot care, application of non-medicated ointments, application of elastic stockings, ace wraps, and compression boots, turning and repositioning patients every two hours, and other associated tasks. • Assisting the patient with elimination including, care of a urinary and condom catheter, use of bedpan, commode, toileting schedules, emptying colostomy bags, urinals and urinary drainage bags. • Carrying out planned patient care measures that provide patient comfort and pain relief such as, coordinating care to provide rest periods, minimizing noise, assisting in bed positioning. • Providing 1:1 monitoring as a patient observer, for patients at risk for injury and also providing the duties of the PCA II for the assigned patient. • Performing simple dressing changes, using aseptic technique. • Assisting nurse/health team members during simple and complex treatments or procedures. • Setting up patients for meals and assisting them with feeding and opening food containers, maintaining calorie counts and recording intake/output measures (I and Os). • Taking patient vital signs, completing intake and output data and daily weights accurately and documenting results completely. • Performing EKGs as directed and according to Hospital and Nursing Department Policy and Procedure. • Performing blood glucose monitoring as directed and according to Hospital and Nursing Department Policy and Procedure. • Documenting patient information in a timely, accurate and complete manner, on patient flow sheets. • Providing end of life care, including comfort measures only and post mortem care. • Collecting, labeling, and sending/transporting specimens to the lab with the appropriate lab slips/requisitions attached. • Initiating patient belongings tracking form and properly securing patient belongings in accordance with hospital policy and in conjunction with the RN. Consults with RN, if patient declines to secure their belongings or have their belongings sent home with family member or friend. • Responding promptly and courteously to all patient call lights and patient requests and seeking assistance from co-workers, as appropriate. • Maintaining patient rooms in an orderly and clean manner and stocking patient rooms, with needed supplies and equipment. • Making routine room rounds to un-clutter, clean and organize patient rooms/areas, when needed. • Checking inventories and restocking/replacing/assembling patient equipment and supplies. • Disposing of trash, soiled linens, medical waste and disposable items, appropriately. • Initiating and performing spot and spill cleaning, as needed. • Performing regular checks per policy and procedure to ensure that equipment is operating properly and has current safety inspections/expiration dates, before patient use. • Identifying broken/expired equipment/supplies, removing from service and notifying Charge RN/Staff Nurse. • Performing initial cleaning/decontamination of equipment/materials, e.g., removing reusable equipment from isolation rooms, and arranging for terminal cleaning, as appropriate. • Identifying emergency/crisis situations and initiating appropriate response actions, within the scope of her/his role. Pay! * Job Summary: As a Behavioral Health Unit Registered Nurse you will care for patients ranging from adolescents to adults with acute and chronic psychiatric disorders. You will work with a team that includes the nursing staff, Mental Health Tech, Emergency Services Providers, and Psychiatric Nurse Practitioner to provide high quality compassionate care for patients in crisis. The Saints campus Emergency Department provides care for Behavioral Health patients in its 24-bed Emergency Department and in the 16-bed secure holding Behavioral Health area. Unit Summary: 40,000 visits annually 24 licensed beds with an Ambulatory Care area Certified Stroke Center Primary Angioplasty Program with transfer protocol Our ER uses a multidisciplinary team of staff including: Registered Nurses, ED Techs, Mental Health Techs, Board Certified Emergency Department Physicians, Physician Assistants, Nurse Practitioners including Psychiatric Nurse Practitioners, Respiratory Therapy, Phlebotomy, Radiology and CT Techs and Security staff. Hours: 36 Hours Night- Every third weekend required. Qualifications: RN with previous Behavioral Health, Psych experience is required. BSN preferred BLS required, ACLS preferred CPI Nonviolent Crisis Intervention with proficiency in verbal de-escalation ED experience not required Current RN licensure in the state of MA in good standing are required BSN is strongly preferred Associate’s degree in nursing may be considered.
