School LPN Nurse Needed in the Bronx 2 days per week, 9am-5pm! Scope of Services Consultant will coordinate and oversee the health care services across the 2 sites. This includes: -Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. This also includes managing all health and nutrition components of the programs in accordance with federal and state regulations, implementing the medication policy including administration of medication, measuring blood pressure, blood sugar and monitor records to ensure compliance with regulatory standards, and function as part of a cohesive team that develops and monitors program operations. Conducting Physical Assessments: -Assist all two center base sites with health related information for each child and review all medical information and charts. -Document visits at all sites for data collection in accordance with all guidelines for health- related child outcomes. -Review child physicals upon enrollment and on a regular basis thereafter to ensure appropriate updates and sustaining of ongoing physical updates. -Ensure all physicals have been completed within the last year. -Ensure the child is up to date with vaccinations. -Provide guidance for the completion of Child Health Care Plans and monitor every six months for update compliance, renewals, or changes in medication and to update doctor/parent permission to continue administering. -Review of any medications that are related to allergies or specific medical care instructions and note this information in the children's file. Job Description The Registered Nurse in the Sexual Health Center provides direct patient care, coordinates treatment plans, collaborates with interdisciplinary teams to ensure quality outcomes, maintains clinical documentation, adheres to safety protocols, and supports patient education and advocacy. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Principal Accountabilities Patient Assessment: Perform systematic, comprehensive, and accurate assessments. Assess potential discharge needs during the initial assessment process. Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. Provide appropriate intervention, reassessment, and documentation. Systematically monitor the maintenance of protocols, standards, and plans of care. Plans and provides individualized patient education. Clinical Skills: Perform nursing-related skills and activities competently. Analyze situations critically. Maintain complete and accurate documentation. Identify and document realistic long and short-term goals. Complete interdisciplinary plan of care record. Participate in interdisciplinary rounds. Evaluate the effectiveness of interventions and revise the plan of care as needed. Recognize and evaluate patient and family responses to therapies and nursing interventions. 3: Professional Skills: Communicate clearly and effectively with all members of the healthcare team. Complete hand-off communication and give complete reports to other caregivers. Demonstrate critical thinking and time management skills necessary for effective patient care. Assume responsibility and accountability for the safe nursing care of assigned patients. Delegate responsibilities appropriately to others. Conducts regular patient rounds to ensure safety and enhance patient experience. Actively participates in daily interdisciplinary huddles to support coordinated care. Utilizes approved clinical communication tools in accordance with organizational standards. Quality and Safety: Adhere to OSHA, Joint Commission, Standards of Nursing Practice supported by NJ BON, professional association, ANA Code of Ethics & Scope of Standards and Practice NJ Board of Health, and hospital standards. Use two patient identifiers and check allergies before medication administration. Follow hand washing protocols and maintain isolation precautions. Properly use and store equipment, checking expiration dates. Demonstrate leadership and function as a resource/mentor. Actively participates in Shared Governance. Outreach and Education: Participate in local, state, and national public health planning initiatives. Assist with monitoring and evaluation of grant-funded projects. Provide education to communities, patients, and families in accordance with grant requirements. Attend external meetings and field-based outreach events. Performs other related duties as assigned. Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Personalized care together with advanced technology is what it means to be Community Strong Responsibilities The ICU Professional Nurse is responsible and accountable for the delivery of patient care utilizing the Nursing Process. She/he implements the Nursing Service standards of care and practice, as appropriate to the ages of the patients served and the areas of clinical practice. Exhibits critical thinking in the assessment, planning, prioritization, delivery and evaluation of patient care. Identifies individualized patient/significant other learning needs and implements appropriate measures to meet needs. Demonstrates knowledge, respect and sensitivity of patient's rights. Serves as a patient advocate. Assures confidentiality of patient information. Maintains effective communication with staff. physicians, patients and families, also within the unit and with other departments. Participates in the development of standards and unit departmental performance improvement activity. Position will work at both the Andrus and Dobbs Ferry Pavilions. Qualifications Bachelor in Nursing preferred. Currently licensed and registered to practice as a Registered Professional Nurse in New York. Current BLS, ACLS , EKG and PALS Certification preferred. 1 year of Critical care or med surg experience preferred.
