JOB SUMMARY The certified nursing assistant is an integral member of the health care team who delivers quality care through collaboration and communication with team members. Gives report when leaving unit, and at the completion of the shift regarding the care that was delivered; any abnormalities that occur during the shift are to be reported as they occur to the RN as well as to the oncoming CNA. Assists in Assessment Identifies the patient per hospital policy before carrying out functions. Assists with data collection for nursing history and physical e.g. vital signs, ht. wt, blood sugar results, functional status and communicates changes to the RN in a timely fashion. Participates in respectful communication with patients, families and coworkers. Provides patient and family information to the registered nurse for coordination of care and discharge planning. Observes skin changes and reports discoloration, abrasions, infestations or skin breakdown. Planning Assists in setting mutually agreed upon realistic individual patient goals. Integrates the RN’s directions re: the patient’s plan of care into the care given to the patient. Gives RN report before the end of the shift regarding the patient’s response to the plan of care issues, and at various times during the shift to report unusual findings e.g. abnormal blood sugar results, vital sign changes, pain etc. Intervention Promotes infection control by using the appropriate hand washing techniques. Follows standard precautions and any other isolation precautions as dictated by the patient’s condition. Looking for a dedicated, compassionate CHHA who will provide personal care services for our patients preparing for end-of-life. You will provide compassionate, one-on-one personal care and companionship to seniors and individuals in their own homes, allowing them to live with dignity, comfort, and independence. Our caregivers work alongside our Lead Care Manager to deliver the highest level of care, from assistance with activities of daily living to emotional support and companionship. You will be a trusted presence in the lives of our clients and their families. WHAT YOU'LL DO Provide on-call support for last-minute caregiver call-offs Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting Provide mobility assistance and safe transfers Prepare meals and assist with feeding as needed Provide companionship and emotional support — build genuine relationships with clients Perform light housekeeping to maintain a clean, safe home environment Follow individualized care plans and document care notes accurately after each visit Report any changes in client condition, behavior, or safety concerns to the office promptly Work alongside the Lead Care Manager on client assessments and first-day introductions Support coverage needs and fill in for unexpected scheduling gaps when needed WHAT WE'RE LOOKING FOR Minimum 1 year of professional caregiving experience with the senior population (required) Must be 21 years of age or older (some exceptions may apply) Own transportation and a valid driver's license + auto insurance Strong communication skills and the ability to follow care plan instructions Familiarity with first aid procedures Current TB test with results Ability to pass a background check Experience with dementia care, Alzheimer's, or memory care is a plus STNA or CNA certification is a plus — not required PHYSICAL REQUIREMENTS Ability to lift up to 50 lbs using safe, ergonomic movements Ability to stand, walk, reach, crouch, and perform hands-on care throughout a shift Physically and mentally able to operate a vehicle and travel between client locations WORK SETTINGS Our caregivers serve clients across a variety of settings, including private residences, independent living communities, assisted living communities, and skilled nursing facilities throughout the APPLY TODAY Join a growing team of dedicated professionals making a real difference in the lives of seniors and their families across the Pittsburgh area. As a hospice Registered Nurse Case manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers. Additional responsibilities for the hospice RN Case Manager will include: Providing nursing care with an emphasis on pain and other management. Supervising Home Health Aides. Referring patients' families to applicable community resources. Maintaining strict patient confidentiality. Providing patient care directives to family members and alternative caregivers.
